**Food safety certified person has a valid Servsafe but doesnot have a valid City of Philadelphia food establishment personnel food safety certification. Application was email.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. Employee added more quarternary ammonia concentration to the sanitizing solution.

**Five unopened packages of cigarettes were observed in a drawer with cash in the customer/prep area. Tobacco ticket was issued and given to PIC.

**Four customers were observed smoking in the establishment. Tobacco tickets were issued and given to PIC.
