1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.

8 Bottles of bar syrups were observed in the handwash sink at the bar area. A handwashing facility may not be used for purposes other than handwashing. *PIC removed syrups at the time of inspection*
8 -The handwash sink in the prep area lacked paper towels
-The handwash sink at the bar area lacked paper towels
-The handwash sink in the customer restroom lacked soap and papertowels
*PIC provided soap and papertowels to all necessary handwash sinks at the time of inspection*
26 Observed PIC's personal care products being stored in the basement employee toilet room. *PIC relocated personal care items at the time of inspection*
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 Small gap observed at the basement trap door. This outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
37 Items were observed directly on the floor in the walk-in cooler; items should be at least six inches off the floor.
45 Equipment (repair):
-defective ice machine observed in the kitchen prep area (not in use)
-defective chest freezer observed in the basement storage area (not in use)
-observed raw wood (2x4) supporting chest freezer in the basement
45 Non-food contact:
-Aluminum foil observed being used to line kitchen equipment and shelving
46 No NSF/ ANSI approved drying rack observed; only raw wood shelving observed to hold clean items. Drainboards, utensil racks or tables large enough to accommodate all soiled and cleaned items that may accumulate during hours of operation shall be provided for necessary utensil holding before cleaning and after sanitizing.
47 -dusty ceiling vents observed throughout the facility
-soiled tubes observed at the 3-basin sink
-bottoms of reach in refrigeration units at the bar area observed with spillage
48 Hot water was lacking at the employee handwash sink closest to mechanical dishwasher due to a loose screw in the faucet- employees were instructed to use the other handwash sink in the prep area (within 5 feet).
49 Observed the handwashing sink in the employee's restroom slow draining.
53 Brooms and dustpans observed idle on the floor in the basement and prep areas. Maintenance tools should be elevated in a designated space when not in use.
53 -Unsealed utility lines with holes observed at the bar area
-Holes observed in ceiling in the bar area
53 General cleaning (premises):
-heavy dust accumulation observed along floor perimeters throughout facility
-food residue/ dust observed behind and underneath kitchen equipment
-dead roaches observed in the downstairs basement and upstairs behind the bar

**Sanitarian emailed a copy of the Philadelphia Food Safety Certificate Application to the person in charge.
14 Observed the chlorine chemical sanitizer bottle empty at the dishwasher.
**A new bottle of sanitizer was placed at the dishwasher and primed at the time of the inspection.
14 Black residue observed on the interior of the ice machine bin around the top back portion.
**The ice machine was unplugged at the time.
49 Observed the handwashing sink in the employee's restroom slow draining.
51 Observed the vent covers in the restrooms with an accumulation of static dust.
53 Observed debris on the floor behind/underneath cooking equipment.
53 Observed an open hole in the floor between the cabinets/tables behind the bar area.
