23 Steaks and burgers are served raw or undercooked to the customer's request; however, a written consumer advisory (on the menu, table tent, or placard) was not provided to the consumer. Management added the consumer advisory to the menu, while on site.
36 The base of the Exit door is an outer opening in the food facility and it does not protect against the entry of insects, rodents, and other animals.
37 Vegetable oil containers are not stored at least 6 inches above the storage room floor.
41 A cup without handle was stored on top of ice used for drinks.
The cup was removed from the ice at the time of the inspection.
Management was told to obtain scoop with handle.
45 Cutting board observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
46 Food facility lacks chlorine sanitizer test strips to monitor sanitizer concentration of ware washing machines (2).
47 Grease and soil accumulations observed on cooking equipment and on the pipes near the fryer.

53 Observed brooms on the floor and leaning against the wall inside storage and utility rooms, instead of being hung on a rack, while not in use.

Defective wall surfaces are not properly sealed at the seams of the wall and floor junctures at the bars and the ice machine areas.

**Eggs were relocated to the bottom shelf at the time of the inspection.
14 Observed pink residue build-up in the soda gun holsters.
**Rubber hostlers were removed, cleaned and sanitized at the time of the inspection.
41 To-go sauce container observed in direct contact with bulk food ingredient (table salt).
**The container was removed from the bulk ingredient at the time of the inspection.
45 Observed non-NSF/ANSI approved drying racks in the kitchen area.
47 Grease accumulation observed along the pipes near the fryer.
49 Observed the handwashing sink in the women's restroom slow draining.
**The handwashing sink waa fixed at the time of the inspection.
53 Observed unsealed pipes near the fryer in the kitchen area.
53 Observed brooms and a dust pan leaning against a wall instead of being hung on a rack while not in use.
**Broom and dustpan were hung on the utility rack at the time of the inspection.
53 - Observed broken glass and debris on the floor around the water pipe of the 3-bay warewash sink at the main bar area.
- Observed a sticky spill under the ice bin in the upper bar area.
**The glass and debris was swept up during the inspection.
**The sticky spill was mopped up during the inspection.
