1 No Food Safety Certified person observed at time of inspection.
**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.**
1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code.
8 The following observations were noted:
-The handwash sink in the food prep area was blocked by food prep table and not easily accessible at all times for employee use.
-Utensils stored in the handwash sink in the food prep area.
-Soap was not available at the handwash sink in the food prep area.
-Paper towel dispenser empty at the handwash sink in the food prep area.
8 Unable to verify if running hot water is available at handwash sink; due to excessive clutter in the food prep area
11 The following observations were noted in the retail area:
- 9 baby formulas (powder and liquid) on display for consumers to purchase with expiration dates of Nov. 2024; Dec. 2024 and Mar. 01, 2025.
- 7 severely dented canned food items stored on retail shelves.
14 The following observations were noted:
-No drain stoppers for the 3-compartment warewash sink.
-No chemical sanitizer available to use on equipment and utensils after washing and rinsing them, and prior to use.
14 Slicer observed with residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
19 Observed cooked food (i.e. pastelillos) was held between 92 -93 degrees F rather than 135 degrees F or above as required.
21 Observed opened commercially processed ready to eat food (i.e. cheese), located in the closed display case in the food prep area, and held more than 48 hours, was not marked with the date it was opened.
26 -Observed insecticides not labeled by the manufacturer for food facility usage, stored on storage rack above food prep table and on floor surface near prepackaged food items in the food prep area.
26 Observed chemical item (i.e. window/glass spray) stored above or on the same surface with food, equipment, and/or single service articles in the food prep area.
36 Observed an unapproved insect control device (i.e. sticky fly strip) hung near the food prep table and cold prep unit in food prep area with potential to contaminate food, equipment, and / or utensils.
36 Observed the store's front door left wide open in the food facility without a screen door; does not protect against the entry of insects, rodents, and other animals.
36 The following observations were noted:
-Flies landing on food contact surface (i.e. wood base cutting board installed onto cold prep unit) in the food prep area.
-Visible physical evidence of rodent activity (i.e. mouse feces and gnawed bag of sliced bread) on the retail shelf in the retail area.
37 Idle prepackage food items stored on direct floor surfaces in the retail and food prep areas.
45 The following observations were noted:
- Non-NSF equipment was observed in the food preparation area (microwave and chest freezer).
- Inoperable garbage grinder food prep area.
- Inoperable cold prep unit food prep area
45 - Garbage grinder observed under shallow hand sink in food prep area.
46 The following observations were noted:
-Warewash sink not clean to sight and/or touch, unclean dishes stored in all compartment, prepackage food and cooked food stored on drainboard of warewash sink.
-No drain stoppers for the 3-compartment warewash sink.
-No chemical sanitizer available to use on equipment and utensils after washing and rinsing them, and prior to use.
-Food store lacks sanitizer test strips to monitor sanitizer concentration.
49 The following observations were noted:
-No toilet room observed, no accessibility provided to basement area to verify the installation of a toilet room and backflow prevention device a main water inlet.
-No mop sink observed.
-Unable to verify running water at handwash sink in the food prep area.
53 Observed excessive clutter throughout the stored and food prep area.
61 Person in charge stated, the "food prep area is not in used to serve the customers"; however, Health Sanitarian observed NSF(i.e. slicer, coffee machine, closed display case, warewash sink) and non- NSF (i.e. microwave, hot-hold unit, chest freezer) equipment and appliances storing food and in use for food preparation.
**Health Sanitarian informed person in charge if the food prep area is not operating to remove all food related equipment and appliances; and submit a Change of Operation application to the Plan Review Unit.**
