1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Corrected on Site: Person arrived during inspection.
8 Observed the following: - No hot water at warewash sink or handwashing sink - No paper towels available at handwashing sink Corrected on Site: Hot water was turned on and paper towels were supplied upon request.
11 Observed the following: - Three dented cans - Five expired Gerber apple juices - Two half gallons of milk - Three gallons of milk Corrected on Site: All items were disposed of upon request.
13 Observed ice marker inside of food prep sink. Corrected on Site: Ice maker was removed upon request.
21 Deli meats and cheeses, prepared in the food facility and held for more than 48 hours, located in the closed display case, was not date marked. Corrected on Site: All foods were date marked upon request.
41 Observed the following: -Knives stored between table edges which are not easily cleaned and sanitized. - Spoon used for coffee stirring in stagnant water.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
8 Paper towel dispenser empty at the handwash sink in the food prep area and restroom. Soap was not available at the handwash sink in the restroom.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use.
20 Mayo was held at 49 degrees F rather than 41 degrees F or below as required.

11 Two containers of Gerbers rice cereal were observed for sale past their sell by date. Discarded.

20 An opened bottle of lemon juice was observed at room temperature despite label stating to refrigerate after opening. Move to deli case.

45 Plastic milk crates are used as food storage. These are not smooth and easy to clean surfaces.
46 Too many items are stored on the drying rack above the sink. Make space for sanitized items to air dry.

53 Broom and mop seen resting on the floor between uses rather than being stored on an elevated rack.
