1 Certified person did not have a Philadelphia certificate. Application given to the person in charge. **Philadelphia certificate needed for LER approval. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
41 Utensil observed in stagnant water (by the grill).
45 Defective equipment observed: pizza oven, walk-in box (basement), two cold prep units, refrigerator, ice cream freezer. **All defective equipment was to be removed or repaired** Non-ANSI certified freezers observed and in use.
54 Exhaust hood filters over the grill was observed with an accumulation of grease.
