- Damaged reach-in refrigerator observed in the kitchen area.
47 Dust accumulation on shelves in the kitchen area.
Dust accumulation and mouse droppings observed on the clothing, toys, and dippers cabinets in 2's classroom.
51 Detached floor tiles in the employee's restroom and 2's restroom.
The hand wash sink faucet need adjustment in employee's restroom.

-Loose and taped base coving in facility.
-Detached big ceiling tile observed in top of the coach in the afterschool classroom.
-Missing ceiling tiles in afterschool and 2's and afterschool classroom.
-Big gaps around the celling heater unit observed in afterschool classroom.
-Water ceiling stains observed throughout the facility.
