14 The 3 basin sink was set up to wash in the middle basin and sanitize in the third basin.
The 3 basin sink was drained and set up to wash, rinse and sanitize during the inspection.
36 -The rear door, an outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. Gaps observed around the closed door.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
The employee put on a cap during the inspection.
41 -Knives and / or cleavers stored between table edges which are not easily cleaned and sanitized.
-Common bowls or food containers observed without handles in direct contact with bulk food ingredients.
-Serving utensils located in the bulk sugar container stored with food contact surfaces exposed to hand contact when retrieved for service.
45 -Non-food grade plastic containers used to store bulk foods and bagged food in the refrigeration units.
45 -The 3 basin sink was observed draining slowly.
-The garbage grinder is installed on a shallow, wall mounted hand wash sink rather than a deep, NSF approved, and stand alone sink.
-An open pipe was observed near the hand wash sink, food prep sink, and the 3 basin sink.
47 Observations:
-Food residue observed on the removeable part of the meat grinder.
The employee placed the part in the 3 basin sink. The entire grinder should be washed, rinsed, and sanitized before further use.
52 -The dumpster was observed open and overflowing rather than tightly closed.
-The used cooking oil dumpster outer surfaces contain accumulated residue.
53 Observations:
-A sealed box of large cans of soy sauce containing scratches, holes and mouse droppings observed in the storage area on a metal dunnage rack.
The person in charge stated that the box was obtained earlier today from a distributor in China Town. The box was emptied and discarded in the dumpster outside. The inspector requested that the cans be cleaned and sanitized before opening.
-Large refrigerator in the customer wait area is not in use.
54 Employee's covered beverage, phone and other personal items were stored on a prep table where broccoli was being prepared.
The employee removed his personal items from the table.
46.963 - Employee Accommodations: Located to Prevent Contamination
(a) Designated eating, drinking and smoking areas. Areas designated for employees to eat, drink and use tobacco shall be located so that food, equipment, linens and single-service and single-use articles are protected from contamination.
(b) Lockers and other facilities. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.

4 Employees' beverage containers were observed in food preparation areas on prep tables.
The observed cups were moved off of the prep tables.
8 The handwash sink in the prep/cooking/warewash area was blocked by dishes and utensils and not accessible at all times for employee use.
The observed items were moved.
13 Food not protected from contamination. Observed containers of raw chicken stored over and next to containers of other raw animal-derived foods in the walk in refrigerator and cold prep refrigerator.
Note:
Food should be protected from contamination by:
2) Separating types of raw animal-derived foods (such as beef,
fish, lamb, pork and poultry) from each other during
storage, preparation, holding and display, except when
these raw animal-derived foods are combined as
ingredients, by doing the following:
(i) using separate equipment for each type or arranging
each type of food in equipment so that cross
contamination of one type with another is prevented.
14 The can opener had an accumulation of food residue on the food contact surfaces.
Inspector requested that the can opener be washed, rinsed and sanitized before further use.

All surfaces should be cleaned and sanitized before further use.
52 -The dumpster was observed open and overflowing rather than tightly closed.
-The used cooking oil dumpster outer surfaces contain accumulated residue.

-A strong sewage type odor in the toilet room.
54 The refrigerator in the customer waiting area is lacking a light.
54 Employee's phone observed stored on the prep table amongst foods being prepared.
The phone was moved.

13 Food not protected from contamination. Observed containers of food used to fill orders in a refrigeration unit in the customer wait area. Containers of food being used to fill orders should be located in the prep area away from customers.
This refrigerator should be moved to the prep area away from customer access.
26 Observed insecticides (a tube) not labeled by the manufacturer for food facility usage, in the prep / cooking area.
The tube was removed from the building.
All pest control should be done by a licensed contractor.
36 The rear door, an outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
- Rear door observed open without adequate screen protection. There are openings on the lower end of the door enabling insects or other vermin to enter.
