1 Facility is engaging in preparing and serving food distribution (repackaging ice from bulk storage bags into Styrofoam cups) without designation a food safety certified person to ensure safe food practices.
** Advised to either (1) discontinue scoop repackaging or (2) submit plans to obtain approval with the appropriate equipment present.
14 Facility is actively scooping ice from bulk ice storage bags with a metal ice scoop with a handle into Styrofoam cups without any methods of washing, rinsing, or sanitizing present.

35 Observed repackaged peanuts without original packaging present in the facility.
36 -Storm cellar door observed propped open at the time of the inspection.
-Side door in the garage and large garage door in the food facility do not protect against the entry of insects, rodents, and other animals.

-Bare wood flooring observed in use inside the walk-in cooler.
-Cardboard lined pallets observed in the middle and garage storage area.

53 -Observed an unfinished ceiling in the basement with exposed support beams and upper level flooring.

53 Mop observed stored inside a mop bucket in the bathroom instead of elevated to promote air drying.
53 Brooms and dustpans observed stored on the floor in the basement and bathroom areas instead of elevated to promote floor cleaning.
54 -Lighting in the walk-in cooler was measured at 31 lux instead of at least 110 lux.
-Lighting in the rear storage area was measured at 16 lux instead of at least 110 lux.
54 Mechanical ventilation not present restroom area.
54 Light in the bathroom observed not shielded from breakage.

** Facility is approved for prepackaged alcoholic beverages, ice, and peanuts distribution only. They have begun scooping ice from its original package into Styrofoam cups. Facility did not obtain approval for this change of operations prior to it occurring.
