1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. Food Safety person arrived in middle of inspection.

26 Observed soap bottles at front handsink without labeling. -person in charge provided labels.

45 Food contact surface of wooden shelving in middle storage room is not smooth, easily cleanable and resistant to pitting, cracking, or scratching.

47 -Sides of cooking equipment, food storage shelving, and food storage containers observed covered with flour and residue in need of cleaning. -Unused waffle irons observed in need of cleaning in rear of facility -Bottom surfaces of reachin refrigerator and freezer units observed with residue present

53 Floors and walls in the kitchen area and front serving area observed with dust and residue accumulation along corners and under equipment.

54 Observed employee personal cell phone on cutting board in rear kitchen at time of inspection. -employee removed immediately and cleaned/sanitized cutting board at time of inspection

54 2 out of 3 bulbs observed out below the kitchen exhaust hood.
