45 Several equipment were not used and out-of-order in the first floor catering kitchen.
45 One milk carte was being sued for elevation in the walk-in cooler located in the basement production kitchen, which is not smooth and easily cleanable.
51 The following observations were noted:
-Openings in the ceiling in the men's restroom area.
-One of four handwashing sinks was not functional in the men's restroom area.
51 Water stained ceiling tile was observed in the men's restroom area.
51 A lidded trash receptacle was not available in the women's restroom area.

14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours.
The slicer was washed, rinsed, and sanitized during the inspection.
42 Clean dish racks observed being stored on the floor.
42 Clean food equipment being stored wet, and not in a manner to allow air drying.
43 Boxes of single-service items were observed stored on the floor of the dry storage closet.
45 - Observed inoperable garbage grinders in the kitchen area.
- Plastic crates were observed being used for elevation in the produce walk-in cooler and in the dry storage area.
47 - Grease accumulations observed on the outer surfaces of the cooking equipment.
- Rubber gaskets on the door of the walk-in cooler were soiled.
53 - Debris accumulations observed under cooking equipment and under the dish washing machine.
- Debris observed under shelving in the dry storage areas.
- Old mouse droppings were observed in the corner between the 2-compartment prep sinks in the kitchen area.
53 Wall behind the dish machine was in poor repair.
