1 The facility's food safety certified handler lacks the City of Philadelphia food safety certificate. Observe a ServSafe certificate only. Advised facility's food safety certified handler to apply for the City of Philadelphia food safety certificate. *Application form issue today 6/17/2024.
14 The chlorine concentration in the sanitizing solution of the 3-bay ware wash sink was more than 200 ppm, rather than 50-100 ppm as required. PIC took corrective action and lowered the chlorine concentration during inspection.
14 -The 3 basin ware wash equipment was not clean on sight and touch. PIC took corrective action and washed the 3 bay sink before to set up for sanitization.
41 Common bowls or food containers observed without handles in direct contact with bulk food ingredients (rice).
43 Single-use utensils (plastic forks) observed facing the wrong way; these items should be inverted to avoid contamination.
45 - Aluminum foil observed on the cooking equipment. - Carboard lining observed on the walls and cooking equipment / kitchen area.
45 - Missing utility rack in the facility. - Domestic rice cooker, microwave, and crock pot.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 - Dust accumulation observed on the shelving in the retail area.

45 Observations: -Domestic rice cooker, microwave, and crock pot. -All wrapping should be removed from the cooking equipment.
51 Observed static dust on vent cover in the restroom area.
53 Broom and dustpan were not hanging on a rack.
54 Observed employees personal items stored throughout the kitchen area. 46.963 - Employee Accommodations: Located to Prevent Contamination (a) Designated eating, drinking and smoking areas. Areas designated for employees to eat, drink and use tobacco shall be located so that food, equipment, linens and single-service and single-use articles are protected from contamination. (b) Lockers and other facilities. Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, utensils, linens and single-service and single-use articles cannot occur.
