- Peak hot water at handwash sink at toilet room near pre-school classroom observed at 80.7 degrees F
- Peak hot water at handwash sink at toilet room at toddler classroom at 78.5 degrees F

Electric 19.9 gal hot water heater was observed in the basement. After waiting 60 mins for hot water to re-generate, hot water has been restored at all handwash sinks. Replace hot water heater to meet peak demands.

6 Hot water is not available in the staff toilet room for proper handwashing. Toilet room was marked out of order and staff was instructed to use the spare rest room between the food prep area and storage room.
8 -Paper towels were not available at the handwashing sink in the food prep area.
-Soap was not available at the handwashing sink in the staff toilet room.
Corrective action: Paper towels and soap were provided.
9 Food facility Person in Charge not able to provide documentation that food in pans inside of the refrigerator and freezer are from an approved source. Facility was approved to provided food prepared by an approved food service program or for parents to bring children meals. However food is being prepared on site.
36 Visible physical evidence of rodent activity (mouse feces) observed on the floor in the storage closet and outside of the storage closet.

Corrective action: All reusable items were bagged up and removed from the facility during the inspection.
19 Food cooked at approximately 9:00 a.m. was observed being held on a table in the prep area without temperature control for 1:00 p.m. meal time.
Corrective action: At sanitarian's request, hot food was placed in the refrigerator at 11:20 a.m. No hot holding equipment available.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the rear storage room area.
Corrective action: Items were relocated during the inspection.
36 Visible physical evidence of rodent / insect activity observed in the establishment. The following were observed:
-Mouse feces in the cabinet in the unused toilet room in the rear, on closet floor where extra cots and chairs were observed stored and on floor between storage racks in the food prep area.
-Flies were observed in the food prep area.
-Dead insects were observed on trap and floor in the prep area and on items in the rear storage area.
39 Moist wiping cloths observed lying on the hand-washing sink and not stored in sanitizing solution.
45 -The refrigerator is holding food above 41 degrees F. The temperature was adjusted during the inspection.
-Base of faucet at handwash/craft sink in classroom is in disrepair.
