1 Certified person did not have a Philadelphia certificate. Application was already mailed.
No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 No sign or poster posted at the handwash sink in the prep area to remind food employees to wash their hands. Sign was posted.
45 Raw wood shelving observed in the dry storage closet was not smooth and easily cleanable.
54 Hood filters have an accumulation of static dust and grease.

8 Dishes and utensils observed in the handwash sink in the prep area. Dishes were removed.
13 Raw chicken was stored above raw seafood in the refrigerator.
21 Opened commercially processed ready to eat food (salami), located in the refrigerator, and held more than 48 hours, was not marked with the date it was opened.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in all refrigeration equipment.
36 Visible physical evidence of rodent / insect activity observed:
- Mouse droppings observed on shelves in the basement.
- Fruit flies observed harboring around onions and potatoes
- Ants observed in the prep area
45 Raw wood shelving observed in the dry storage closet was not smooth and easily cleanable.
51 Lidded trashcan was not provided in the women's restroom for the proper disposal of sanitary napkins.
51 Water stained ceiling tile and dust accumulation on vent cover observed in the women's restroom.
53 General cleaning is needed in the basement to declutter items to prevent pest harborage.
54 Lights in the warewashing/prep area were not shielded from breakage.
