8 No running cold water was observed at the handwashing sink located in the meat cutting room. The person in charge took corrective action and cold water was functional before the end of the inspection.
45 Unused equipment was observed in the poultry service area.
47 The following observations were noted: -Exterior surface cleaning was needed around washing station in meat cutting room due to residue. -The floors of the walk-in coolers/ freezers were in need of cleaning due to residue and debris. -Rust around shelves in multiple refrigeration units. -Dust around the fans in the chicken walk-in cooler was in need of cleaning.
49 A slight leak was observed coming from the lower pipe of the 3-compartmnet warewashing sink located in the kitchen area.
51 The restroom door was observed damaged from the inside.
53 Floor cleaning was needed under and around equipment in several areas of the facility due to debris.
54 The hood filters were observed with an accumulation of static.

45 The cold water faucet of one handwashing sink located in the rear preparation area was in need of general repair due to difficulty turning on.
45 Raw wood pallets were observed being used for elevation in the walk-in coolers/freezers.
47 The following observations were noted: -Exterior surface cleaning was needed around washing stations in all preparation areas due to residue. -The floors of the walk-in coolers were in need of cleaning due to residue and debris. -Rust around shelves in multiple refrigeration units.

8 The hot water faucet of the handwashing sink located in the meat preparation area was not functional and in need of repair. NOTE: Hot water was available at other stations in the facility.
36 Mouse activity was observed around the perimeters of the floor in the food preparation areas, warewashing areas, and storage areas.
45 The following observations were noted: -An unused, non operational display case located in the poultry retail area. -The faucet of the 3-comaprtment warewashing sink located in the poultry area was in need of repair.
45 The following observations were noted: -Raw wood pallets were being used for elevation in the walk-in coolers/freezers. -Cardboard was being used to line the interior surfaces of refrigeration units in several areas.
47 The following observations were noted: -The floor of all walk-in coolers/freezers were in need of cleaning due to food debris/residue. -The interior lower surfaces of several display cases and reach-in refrigerators were in need of cleaning due to food debris.
53 Floor cleaning was needed under equipment and around the perimeters of the floor in multiple areas due to debris and mouse droppings.
