36 Gaps observed in the trap door in the basement storage area which is an outer opening in the food facility and does not protect against the entry of insects, rodents, and other animals.

36 - Observed mouse droppings on the floor perimeters in the downstairs prep, refuse room, and upstairs kitchen/prep areas.
37 - Observed food items not stored at least 6 inches above the floor in the walk-in cooler.

1 The Person in Charge did not demonstrate adequate knowledge of the Philadelphia Food Code. **Sanitarian recited the 5 foodborne illnesses to the person in charge.
8 No sign or poster posted at the handwashing sink in the kitchen area to remind food employees to wash their hands.
13 Raw chicken was stored above raw beef and ready to eat foods in the walk-in cooler. **Raw chicken was relocated at the time.
13 Observed no splash guard in between the handwashing sink and prep table in the kitchen area.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 100 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. **Additional tablets were added to the solution to increase the concentration.
20 Food (garlic, chicken, shaved beef, sliced veggies) were held at 43.2 - 47.7 degrees F rather than 41 degrees F or below as required. **Effected foods were discarded at the time of the inspection.
33 Food (Beef) observed thawing at room temperature in the prep sink.
36 Visible physical evidence of rodent activity (mouse droppings) observed in the trash area, lower prep area and upper closet containing tanks.
