1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Corrective action: an application for the department issued certificate was emailed to the facility.
47 Ice accumulation observed in the chest freezer in the serving area.
49 Hot water handle on the dump sink in the serving area was not functional.
53 Broom, dust pan, and squeegee observed stored on the floor next to the mop sink rather than elevated to promote floor cleaning.
53 Unsealed utility lines observed on the ceiling above and in the floor below an electrical panel next to the reach in fridge in the employees only area.

14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 500 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. More water added to Quartnery solution to derive 200 ppm.
