2 Food safety certified person could not demonstrate adequate knowledge of the foodborne illnesses deemed reportable and the symptoms that would require employee exclusion.
6 -Food employees observed donning single use gloves without a prior hand wash. -Food employee did not follow proper handwash procedures. Food employee did not wash hands between changing tasks.
8 The handwash sink in the food prep area had a tray of money in it and was not accessible at all times for employee use. Corrective action: Tray was moved and rice cooker was moved away from the sink also.
13 Nozzles/lids on condiment squeeze bottle containers observed soiled. Black residue observed.
20 -Potentially hazardous food was held at 44.8 to 45.9 degrees F rather than 41 degrees F or below as required. -Cheese held at 59 degrees F, rather than 45 degrees F or below as required. Corrective action: Person in charge was instructed to discard the tomatoes and lettuce at the 4 hour mark. Lettuce was still in use when Sanitarian returned 20 minutes later. Lettuce was discarded at Sanitarian's request. Cheese discarded. Additional ice was added to compartments holding food. Note: To keep food at 41 degrees F or lower, containers should not be filled to the top and containers should be surrounded on all sides with ice.

6 Water at the handwashing sink was 72.9 rather at than 100 degrees F. Corrective action: Water temperature was adjusted during the inspection.
13 Cell phone and ear buds observed stored in carton of eggs. Corrective action: Items removed. Eggs discarded.
14 There are not enough utensil on the cart to replace at 4-hours intervals. Corrective action: Service is ending, and owner ensured that there will be at least three of each of the following utensils tomorrow - knives, tongs for raw meat, tongs for cooked meat, falafel scoop, etc.
20 -Potentially hazardous food was held at 52-54 degrees F rather than 41 degrees F or below as required. Corrective action: Container of fish not stored in contact with ice was discarded. -Eggs held at 66.9 degrees F, rather than 45 degrees F or below as required. Corrective action: Eggs stored in the food prep area were discarded. Note: Food containers must be in contact with ice in order to for the food to remain cold. Please do not stack food containers.

Corrective action: Items discarded. Note: Eggs and sauce must be stored at the proper temperature or labeled with the date and an appropriate disposal window.
