1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.
1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. -city of Philadelphia Food Safety Certificate Application will be emailed with inspection report.
8 No sign or poster posted at the handwash sink in the prep area to remind food employees to wash their hands.
8 The handwash sink in the prep area does not have single use towels, continuous towels, or air drying device.
8 Hot water not readily available inside employee restroom during to nozzle/fixture missing. Only cold water was available at time of inspection.

53 -Hard to clean flooring observed throughout the rear food prep/ware wash area. - Damaged wall tiles in the rear food prep area. - Damaged floor tiles in front and back of facility. -Danaged walls above prep area, appearance of previous ceiling or water leak in ceiling

54 If necessary to keep rooms free of excessive heat, steam, condensation, vapors, obnoxious odors, smoke and fumes in a food facility, mechanical ventilation of sufficient capacity shall be provided. -no mechanical ventilation hood was present for current cooking practices including frying of falafel, gyro meat cooking device is use, raw meats (chicken) cooked in table top convection oven.
