1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **ServSafe was expired. The Application for the city of Philadelphia certification was sent.
36 Visible physical evidence of rodent (Mouse Droppings) activity observed in the closet area.
45 The following was observed: - 1 unused paper towel dispenser - Domestic Cabinets in all rooms

8 The following were observed: -Paper towel dispenser empty at the handwashing sinks in the food prep area and Jellyfish Classroom -Soap was not available at the handwashing sink in the Jellyfish Classroom -No sign or poster posted at the handwashing sink in the Jellyfish Classroom to remind food employees to wash their hands -Hot water does not reach peak demand in Turtle's Classroom restroom area. Hot water observed at a temperature of 95-96 degrees F. **PIC took corrective action and provided paper towel and soap in classrooms where necessary, and increased mixing valve for hot water to reach peak demand. Hot water observed at a temperature of 100-100 degrees F during time of inspection.
26 Chemicals were stored on the same shelf with food in the food prep area. **PIC took corrective action and stored chemicals in a chemical only designated area during time of inspection.
36 Visible physical evidence of insect activity (dead roaches/water-bugs) observed on glue trap under shelving unit in food prep area. ** Dead or trapped birds, insects, rodents and other pests shall be removed from control devices and the premises at a frequency that prevents their accumulation, decomposition or the attraction of pests. **PIC took corrective action and discarded glue trap during time of inspection.
47 The following were observed: -Debris/static dust accumulation on lower exterior surfaces of shelving units in food prep area -Food debris/residue on interior surfaces of microwave unit
48 Hot water does not reach peak demand in Turtle's Classroom restroom area. Hot water observed at a temperature of 95-96 degrees F. **PIC took corrective action and increased mixing valve for hot water to reach peak demand. Hot water observed at a temperature of 100-100 degrees F during time of inspection.
