47 -Food debris observed on floor surface under racks in walk-in cooler. - Food debris observed on bottom surface of reach-in freezer and refrigerator. -Kick plates observed with dust accumulation on reach-in units in kitchen.

53 Broom and dustpan observed not properly stored and elevated off of the ground when not in use.

Suite 649 has visible floor gap along wall perimeter. Suite 637 has wall damage behind door. - Art centers observed not in use. 5th floor art center is used for storage.

6 - Food employees observed donning single use gloves without a prior hand wash. * Inspector redirected person in charge to properly wash hands prior to putting on new gloves.
8 -Hot water observed at 136 degrees F at hand wash sink in kitchen area. - Hot water observed at 120 degrees F in day care restroom. * Water is too hot to properly wash hands.

36 Visible physical evidence of rodent (Mouse droppings) activity observed in the kitchen storage area under storage rack and behind hot water heater.

53 The following observations were made: - Debris under storage racks in kitchen area. - White pipe hanging in floor drain. Air gap needed. - Debris under reach-in refrigerator, under lockers and microwave table in dining area.
53 - Heavy grease accumulation on flooring surface behind and under hot hold unit. - Dust accumulation on ceiling vents in kitchen and office area.
