**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**PIC was issued an application for the City Certificate during time of inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
36 An unapproved insect control device located in bar area with potential to contaminate food, equipment, and / or utensils.
**PIC took corrective device and discarded insect control device during time of inspection.
37 Food is not stored at least 6 inches above the floor in walk-in cooler/freezer.
41 The following were observed:
-Food dispensing utensil in the bulk ingredient containers observed in direct contact with food and not having handle stored above the top of the food
-Food dispensing utensil observed stored in stagnated water.
**Storage in a container of water. During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 58°C (135°F) and the container is cleaned at frequency.

45 The following were observed:
-Loose faucet handles on handwashing sinks in food prep area and near ware-washing area
-Cold prep unit and under-the-counter refrigeration unit under/across from exhaust hood does not maintain proper cold holding temperature. Ambient air temperature observed at 54-65 rather than 41 degrees or below. However, potentially hazardous food observed in proper cold holding temperature range.
**PIC was instructed to store potentially hazardous food in refrigeration units with proper cold holding temperature until serviced by maintenance.
47 The following were observed:
-Food debris around perimeters of floors in walk-in cooler/freezer
-Static dust on fan guards in walk-in cooler
-Ice accumulation around perimeters of ceiling and door in walk-in freezer
