1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. The application for the Philadelphia food saftey certificate was emailed to the facility.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 0 ppm, and not 50-100 ppm as required. Corrective action: Person In Charge agreed to sanitize all wares in the 3 bay sink.

6 Maximum hot water temperature in the customer toilet rooms was observed at 71.3 degrees F rather than 100 degrees F as required. Corrective action: the toilet rooms were marked out of order.
20 Diced tomatoes were held at 45 degrees F rather than 41 degrees F or below as required in the walk-in cooler in the food prep area. Corrective action: The person in charge used a heat gun to remove ice from the mechanical components of the walk-in, which increased the airflow from its fans.

19 Beef was held at 120 degrees F rather than 135 degrees F or above as required. Corrective action: the beef was reheated on the grill.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods, in the display fridge in the serving area, without written procedures or documentation to verify disposition of food. Corrective action: Person In Charge created a time table to document time and temperatures for the food in the display fridge.
