8 -The rear hand sink was lacking a trash can nearby.
A can was placed near this sink.
-A string was tied to the faucet of the rear warewash area hand sink to hold the door open.
The door was closed during the inspection.
41 Knife stored under equipment on the counter which is not easily cleaned and sanitized.
The knife was placed in the 3 basin sink to be washed, rinsed, sanitized and air dried.
45 -Plastic crates used to elevate items rather than NSF approved dunnage racks in the walk in freezer and in the rear storage area.
-Cardboard on the floor in the walk in cooler is not smooth and easily cleaned.
45 -The detergent automatic dispenser part is damaged and requires a replacement. Presently, staff are manually dispensing the detergent at the 3 basin sink.
-An under counter refrigerator is not in use in the rear prep area.
47 Observations:
-Accumulated and hanging static dust observed on inner surfaces of both sides of the box exhaust hood.
-Dust on surfaces in the prep areas.
54 Accumulated static dust observed hanging from inner surfaces in the box exhaust hood on both sides over equipment.
54 Employee's hand bags stored on boxes on storage shelf and boxes of beverage syrups.
The bags were placed on the designated hooks.

4 Employees water bottles (2) and orange juice bottle were observed in food preparation areas.
The observed bottles were discarded.
8 Paper towel dispenser empty at the handwash sink in the front service / prep area.
A role of paper towels was provided.
8 -The front and rear hand sinks were lacking trash cans nearby.
They were provided.
-A string was tied to the faucet of the rear warewash area hand sink to hold the door open.This blocks access to this sink.
The door was closed during the inspection.
14 Pink slime observed on the interior of the ice machine bin around the cover for the ice release area.
The person in charge turned off, emptied, washed, rinsed, sanitized and air dried the ice machine. The ice was discarded.
20 Potentially hazaardous food was held at temperatures higher than 41 degrees F rather than 41 degrees F or below as required in the salad prep cold hold refrigerator. The ambiant air was probed at 58 degrees F.
See the temperature chart below. Do not use this cold prep refrigerator until it can maintain foods at a temperature of 41 degrees F or lower.
All of the potentially hazardous foods were discarded in this unit since there was no time temperature log readily available.
36 Visible physical evidence of rodent activity (mouse droppings) observed in the front service / prep area.
45 Equipment is not in good repair:
-The small cold prep refrigerator in the front prep/service area is not maintaining foods at 41 degrees F or lower.
-A light is not working in the front box exhaust hood.

-Dust and debris observed on floor perimeters throughout.
-Static dust observed on surfaces in the rear warewash / prep / storage areas.
-Dust, debris and residue on the floor perimeters in the cage area.
54 One of four lights is not working in the box exhaust hood over the grill.
54 Accumulated static dust observed hanging from inner surfaces in the box exhaust hood on both sides over equipment.
54 Employee's hand bag stored on a box on the storage shelf.
The bag was placed on the employee's belongings rack.
54 The rear warewash / food prep / storage area room temperature was probed at 79 degrees F.
The air conditioning is not cooling this area adequately.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
The person in charge has a Philadelphia Food Manager Certificate that expired January 24, 2022. Another Philadelphia Food Safety Certified Manager arrived during the inspection.
8 -The handwash sink in the rear warewash area was blocked by a rope tied to the spout and door and not accessible at all times for employee use.
-No trash can provided near the rear warewash area hand sink.
The rope was removed and a can was placed near the hand sink.
13 A tray of chicken was not protected from contamination in the walk in refrigerator.
The tray was covered during the inspection.
35 Prepackaged muffins are not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
41 Common bowls or food containers observed without handles in direct contact with food ingredients in the cold prep refrigerator.
The containers were moved.
45 Plastic crates used to elevate items in the walk in freezer.
47 -Static dust observed on the walk in cooler fan cover.
-Accumulated static dust observed inside the exhaust hoods over the cooking and prep area.
-Static dust on electronics in the rear warewash/desk area.
-Static dust and debris on the shelves in the cage area.
51 Handwash reminder signs are not displayed in the common toilet rooms near the restaurant.

-Dust, debris and residue on the floor perimeters in the cage area.

20 The larger cold prep refrigerator is not keeping foods at 41 degrees F or lower. The ambiant air was over 50 degrees F.
-Potentially hazardous food was held above 41 degrees F rather than 41 degrees F or below as required.
-Cheese was held above 45 degrees F, rather than 45 degrees F or below as required.
The potentially hazardous foods were discarded. Do not use this unit until it can maintain a temperature of 41 degrees F or lower.
36 Visible physical evidence of rodent (mouse droppings) activity observed in the warewash area in the rear under the 3 basin sink and on and around the grinder.
37 Food is not stored at least 6 inches above the floor in the walk in cooler.
45 -The larger cold prep refrigerator is not maintaining a temperature of 41 degrees F or lower. It is located next to the oven. Do not use this unit until it can maintain a temperature of 41 degrees F or lower.
-The garbage disposal was not working.
47 -Static dust observed on the walk in cooler fan cover.
-Accumulated static dust observed in the large box hood over the oven.
-Dust and mouse droppings observed on the outer surface of the garbage grinder in the warewash area.
51 Handwash reminder signs are not displayed in the common toilet rooms near the restaurant.

-An under counter refrigerator that is used for dry storage only in the prep area.

Employees' belongings were moved to the hooks in the prep area.

A bottle of sanitizer was obtained during the inspection.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food.
Individual packets of cream observed on the counter without ice or a time/temperature log.
The bowl of cream packets were discarded.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in the small cold prep refrigerator.
