1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Application emailed.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
45 -Counter is not sealed to shaker cabinets on the right side.
53 Missing ceiling surfaces above fire suppression system.
58 Prior to the start of renovations a completed Plan Review Application with proper plans, drawn to scale, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the Office of Food Protection. 1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.

1 -The Person in Charge did not demonstrate adequate knowledge of sanitizer concentration levels and food born illnesses.
8 A handwash sink is not conveniently located for customers.
13 Raw eggs were stored above ready to eat foods in the walk-in cooler.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Observed employee washing cups and utensils in handwash sink and not in warewash sinks as required. Employee took corrective action and placed items in warewash.
14 -Ice machine observed located under 3 basin warewash sinks is prohibited. -Cut produce operation observed without a food preparation sink as required.
20 Ham and cheese and spinach and cheese pastries was held at 59 degrees F rather than 41 degrees F or below as required. See discard list.
21 -Deli meats and cheeses, opened commercially processed ready to eat food, located in the walk-in cooler and cold preparation refrigerators, and held more than 48 hours, was not marked with the date it was opened. -Cut tomatoes potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the cold preparation refrigerator, was not date marked.
34 Temperature measuring devices for ensuring proper temperatures of equipment are not available or readily accessible in refrigeration equipment.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.
35 Prepackaged pasta sauces is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
45 -Counter is not sealed to wall. -Counter is not sealed to shaker cabinets on the right side.
45 -Ice machine is not NSF/ANSI approved. -A plastic storage shelf observed is not NSF/ANSI approved.
46 Food facility lacks sanitizer test strips to monitor chlorine sanitizer concentration.
49 -Back flow device was not observed in cellar and will need to be located for building. -Back flow devices were not observed on espresso machine.
50 Improper condensate drainage observed for ice machine and coffee machine. Ice machine condensate was being drained directly into a bucket and coffee machine condensate was being drained into an open pipe Y pipe and not into an indirect drain with an air gap.
53 Missing ceiling surfaces above fire suppression system.
53 Dead bugs and debris observed in basement utility area.
56 A "No Smoking" sign was not observed.
