8 A bucket of utensils observed in the handwash sink; a handwash sink should only be used for handwashing and shall be maintained so that it is accessible at all times for employee use. *PIC removed utensils at the time of inspection*
36 Observed window was open at the time of inspection. Outer opening in the food facility may not protect against the entry of insects, rodents, and other animals.
37 Multiple food items (bucket of chicken, dressings, tomatoes, and lettuce) are not stored at least 6 inches above the floor.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.

41 Observed utensils being stored in a container of room temperature, murky water: During pauses in food preparation or dispensing, food preparation and dispensing utensils may be stored in a container of water if the water is maintained at a temperature of at least 135 degrees F.
45 Observed aluminum foil being used to line shelving and kitchen equipment. Surfaces of equipment that are nonfood-contact surfaces, but are exposed to splash, spillage or other food soiling or that require frequent cleaning, shall be constructed of a corrosion-resistant, nonabsorbent and smooth material.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Person In Charge (PIC) was not present at beginning of the inspection, but arrived later during the inspection.
6 The water temperature at the handwashing sink was measured at 85 degrees F, below the required 100 degrees F.
**PIC made adjustment to the hot water tank, and the hot water temperature was measured at 101 degrees F after this corrective action taken.
20 Food (Lettuce, tomato, cucumber) was held at 48 degrees F rather than 41 degrees F or below as required in the cooler box near the hot holding unit. No ice observed inside the cooler box.
**PIC added bags of ice to the cooler, temperature measured at 40-41 after the corrective action taken.
