1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Person arrived 10 minutes into the inspection
8 The following was observed: -No paper towel observed in the dry storage handwash sink -Handwash sink had old debris, it was unclean after usage -Hot water was not available at all the sinks.
37 Food is not stored at least 6 inches above the floor in the dry storage area
45 Door gasket on the chest freezer observed not sealing properly.
45 The following was observed: - Milk crates being used for food elevation - Raw wood shelf behind the counter storing food
47 Ice accumulation in the chest freezers
48 Water not under pressure during inspection. No hot water during the inspection.
53 Utility line gabs observed in the food prep area
53 Ceiling tiles observed not smooth and easily cleanable

8 The following was observed: -Handwash sink was blocked off by a bike and Other equipment. -No paper towel observed in the dry storage handwash sink -Handwash sink had old debris, it was unclean after usage
13 A bicycle was observed in the food preparation area.
14 The 3-bay sink observed uncleaned or sanitized after usage.
35 Prepackaged food in the chest freezer is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 Visible physical evidence of rodent / insect activity observed in the facility. Cockroaches and flies were observed at the rear and front areas of the facility.
37 Food is not stored at least 6 inches above the floor in the dry storage area

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Food safety certified person arrived about 15 minutes into inspection.
6 Food employee did not follow proper handwash procedures. Food employee did not wash hands between changing tasks. Food employees observed donning single use gloves without a prior hand wash. Food employees were observed cutting meat. Person in-charge was instructed to take immediate corrective action.

11 Severely dented, swollen, distressed canned items observed in food retail area. **PIC took corrective action and discarded dented cans during time of inspection.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles in the food retail area. **PIC took corrective action and moved chemicals to a chemical only designated area during time of inspection.
37 Food is not stored at least 6 inches above the floor in food prep area.
45 The following were observed: -Leak at 3 bay ware-washing sink -One non-operational reach-in refrigeration unit being used as dry storage -Damaged kickplates on reach-in refrigeration units -3 bay ware-washing sink improperly mounted to wall -Non NSF/ANSI chest freezer units
46 Food facility lacks proper sanitizer test strips to monitor sanitizer concentration.
47 The following were observed: -Light food debris on interior surfaces of reach-in refrigeration units in food retail area -Light food debris exterior surfaces of shelving units in food retail area -Ice accumulation on interior surfaces of chest freezers

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Fatu Corneh with servsafe certificate arrived towards end of inspection.
9 Food facility Person in Charge not able to provide documentation that (groundnut sauce etc) foods are from an approved source. Person in-charge was instructed to pull all identified food items from shelves.
13 Provide a splash guard between the handwash sink and 3-compartment sink.
13 Big chunk of meat approximately 25 pounds was observed being cut in the food preparation area. Facility lacks a dedicated food preparation sink as required. Person in-charge was instructed to desist from food preparation unless a food preparation sink with an indirect connection is installed.
14 Bleach sanitizer being used is not approved by NSF for use on food contact surfaces. Person in-charge was instructed to get an approved sanitizer.
49 Backflow prevention device was not observed on the main water line.
53 Mop head was observed improperly stored.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. Person in-charge is required to apply for and obtain a City issued Food Safety Certificate.
8 The handwash sink has a pot inside it and not accessible at all times for employee use. Pot was removed.
9 Food facility Person in Charge not able to provide documentation that (smoked fish, other ethnic foods) foods are from an approved source. All identified food items were pulled from shelves and refrigeration units.
13 Provide a splash guard between the handwash sink and 3-compartment sink.
13 Uncovered palm oil was observed the rear area. Oil was covered.
14 Sanitizer in use is not an NSF approved agent for food contact surfaces. New sanitizer was provided.
14 Facility is required to provide stoppers for all sinks. Only one stoppre was observed at time of inspection. Inspector demonstrated how to prepare sanitizing solution and properly set up 3-comartment sink.
35 Prepackaged (ethic food items) food is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts.
36 A fly was observed in the facility.
36 * outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. Rear exit door requires a door sweep to protect against entry of rodents.
