1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. At the beginning of the inspection, there was no Philadelphia Certified Food Manager Present. He arrived within the alotted time.
8 Bags of rolls observed in the handwash sink in the food prep area. They were removed.
26 Observed insecticides (Hot Shot Spray) not labeled by the manufacturer for food facility usage, in the food prep area. The can was removed from the prep area. Note: All pest control should be done by a licensed exterminator.
34 Temperature measuring devices (probe thermometer) for ensuring proper temperatures of food are not available or readily accessible.
41 A spoon was observed stored inside of a cup of stagnant water in the coffee prep area.
45 -Raw wood shelves, which are not smooth and easily cleanable, being used to support equipment in the food prep area and the basement storage area. - Plastic crates are used for elevation in the food prep and storage area. - Cardboard was observed used to line shelving in the retail area.
45 - Non-NSF chest freezers observed.
47 -Cardboard liners observed with oil stains and food debris. -Frost accumulation observed in the chest freezers.
53 -Damaged,water stained porous ceiling tiles observed in the rear storage and toilet room areas. - Missing top stair makes entry into the basement hazardous. - Basement floors and walls are not in good repair.
53 The premises of a food facility shall be free of items that are unnecessary to the operation or maintenance of the facility, such as equipment that is nonfunctional or no longer used, and litter. -Items unnecessary for the operation of the business was observed stored in the basement. - Dirt and debris was observed on the floor throughout the basement storage area.

6 Staff is unable to properly wash their hands after using restroom due to the lack of hot water of at the restroom hand sink. Hot water temperature was observed at 84 degrees F.
11 Molded strawberries were observed inside of the refrigerated display case. -Strawberries were removed for disposal
14 Old encrusted food was observed along the interior of the slicer. * Slicer must be disassembled and properly wash, rinsed and sanitized atleast once every 4 hrs.
34 Thermometer inside sandwich prep unit was not measuring accurate ambient air temperatures.
36 Unapproved insect control device was observed in the food prep area. - Device was removed and disposed off.
36 Several large flies were observed in the food prep area.
39 Moist wiping cloth was observed stored on the 3 basin sink. Moist cloths must be stored inside of a container with sanitizing solution.
41 A spoon was observed stored inside of a cup of stagnant water in the coffee prep area.
45 -Raw wood shelves, which are not smooth and easily cleanable, being used to support equipment in the basement storage area - Plastic crates are used for elevation in the food prep and storage area. - Card board was observed used to line shelving in the retail area.
45 -The 3 basin sink is lacking a faucet and spout on the left side. The sprayer must be held to fill up the sanitizer basin. -The garbage grinder is installed on a sink that is not at least 10 inches in depth. -Non-NSF or ANSI approved toaster and chest freezers observed. - Unapproved colander is used for washing produce at the 3 basin sink. * Colander must fit across basin of the sink
45 Handwashing sinks are not properly sealed to the wall.
49 Handwashing sink inside of the restroom is not properly draining.

13 Produce: tomatoes, lettuce, onions are washed and prepped in this facility. There is no dedicated, NSF approved, indirectly drained, stand alone, at least 10 inches in depth food prep sink equipped with a catch basin. There is no large metal colander available for washing the produce.
34 Temperature measuring devices for ensuring proper equipment temperatures are not calibrated and/or functioning properly in the deli display case. Another thermometer was placed in the deli display case.
45 -Raw wood shelves, which are not smooth and easily cleanable, being used to support equipment in the deli area around the 3 basin sink. -Cardboard and other liners are in use in the deli display case and retail refrigerators.
45 -The 3 basin sink is lacking a faucet and spout on the left side. The sprayer must be held to fill up the sanitizer basin. -The garbage grinder is installed on a sink that is not at least 10 inches in depth. -Non-NSF or ANSI approved toaster and chest freezers observed.
45 The small mesh strainer is not adequate for the washing and rinsing of produce. A large metal colander that can sit over a basin in the 3 basin sink is required if this facility is approved for a variance.
46 Non-commercial drain rack in use at the 3 basin sink. The commercial drain rack is being used for storage of food items instead of air drying dishes, equipment and utensils.
47 -Oil stained cardboard lining and surfaces observed on the retail shelves.
49 There is no dedicated, NSF approved, indirectly drained, stand alone, at least 10 inches in depth food prep sink equipped with a catch basin.
52 Excess equipment stored outside. No covered cans observed in the outside trash storage area.
53 -Excess items stored in the basement storage area. For example, wood and various other items. -Dust and debris on basement floor perimeters.

1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. There was no Food Safety Manager with a Philadelphia Food Safety Certificate at the beginning of the inspection. He arrived within the allotted time.
8 The handwash sink in the deli prep area was blocked by a chair and not accessible at all times for employee use. The chair was moved and then placed back in front of the sink during the inspection.
9 Food facility Person in Charge not able to provide documentation that beef patties are from an approved source. The observed patties were removed from the store.
11 Expired baby teething cookies observed on the retail shelf. The box was removed to discard.
14 Slicer has an accumulation of dried food residue on the inner food contact surfaces and is not being taken apart, washed, rinsed and sanitized at least every 4 hours. The person in charge took apart the slicer and began to clean the slicer parts during the inspection. Wash, rinse, sanitize and air dry the slicer parts before further use.
26 -Chemicals (cleaning solutions, Glade air freshener and hand sanitizer) were stored above or on the same shelf with food, equipment, and/or single service articles in the deli prep area. -Employee medicines and personal hygiene products stored in the deli prep area, and were not stored to prevent the contamination of food, equipment or utensils. The observed items were moved in the deli prep area. -Personal hygiene products stored in the cashier area on shelves, and were not stored to prevent the contamination of food, equipment or utensils.
35 Prepackaged and repackaged food is not labeled properly with the name of product, ingredient statement, net weight, distributed by statement and/or nutritional facts. For example, bags of cereal, snack cakes, repackaged dog and cat food. The bags of cereal were removed from the retail shelf.
37 Bottle of olive oil stored on the floor under the 3 Compartment Sink rather than 6 inches off of the floor. The bottle was moved.
45 Raw wood shelves, which are not smooth and easily cleanable, being used to support equipment.
45 -Unapproved not NSF or ANSI approved cooking equipment observed in the deli prep area.: Hot dog grill, microwave, air fryer, toaster, blenders (2), and electric hot plate. -The chest freezer is not NSF or ANSI approved. -Water is draining into the bottom of the deli display case where prepackaged beverages are stored.
47 -Soiled cardboard lining and surfaces observed in the retail refrigerators.
53 Debris and dust observed on the floor perimeter behind the chest freezer.
53 The broom and dust pan were stored on the floor rather than hanging.
53 -Water stained porous ceiling tiles observed in the rear storage and toilet room areas. -Missing top stair makes entry into the basement hazardous.

74 The establishment is selling tobacco without a valid Philadelphia Tobacco Retailer Permit. Observed tobacco and vape products on the cashier area retail shelving. All tobacco, vape and e-cigarrette products must be removed from the store. The products can not be stored anywhere in the facility. A tobacco ticket was issued during the inspection.
