36 Observed the front door propped open at the time of inspection, which is an outer opening in the food facility and does not protect against the entry of insects, rodents, and other animals.

4 Food employee observed eating food in the kitchen area at a table used for food prep. Corrective action; Sanitarian asked food employee to discontinue eating in food prep areas during the inspection.
8 No sign or poster posted at the handwashing sink in the employee restroom area to remind food employees to wash their hands.
13 - Observed multiple raw and RTE food items in the walk-in cooler without proper coverings to prevent contamination. Corrective action; PIC placed proper coverings on food items during the inspection.
20 Eggrolls and sauces were held at 45 degrees F rather than 41 degrees F or below as required inside of the front reach-in refrigerator. Corrective action; PIC adjusted the temperature of the reach-in refrigerator unit to derive the proper cold holding temperature of 40 degrees by the end of the inspection.
21 Observed ready to eat food (egg rolls) prepared in the food facility, located in the front reach-in refrigerator, were not date marked. Corrective action; PIC placed the date on the food items during the inspection.
36 - Observed mouse droppings on lower shelving surfaces storing food prep buckets and cooking equipment (pots) in the prep area. - Observed mouse droppings on shelving surfaces next to and underneath a rice cooker in the prep area. - Observed mouse droppings on shelving surfaces storing bulk ingredients in the rear storage area. - Observed mouse droppings on floor perimeters in the office/beverage storage room and next to the rear exit door.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. **Person in charge arrived 5 minutes into the inspection.
13 Raw animal foods (shelled eggs) were stored above ready to eat foods in the walk-in cooler. **The shelled eggs were relocated at the time of the inspection.
35 Working bulk food ingredient storage container was not labeled with the common name of the food.
36 Observed the back screen door not fully sealed which currently does not protect against the entry of insects, rodents, and other animals.
37 Food is not stored at least 6 inches above the floor in the walk-in cooler and near the bulk ingredients shelf.
