1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
No food safety certified person on site.
Corrective action: Food safety certified person arrived during report writing.
2 No food safety certified person available to demonstrate knowledge of the foodborne illnesses deemed reportable or the symptoms that would require employee exclusion.
Corrective action: Arriving food safety certified person was able to demonstrate knowledge of most illnesses and symptoms.
8 Soap was not observed at the handwashing sink in the Fireflies classroom.
Person in charge was instructed to take immediate corrective action.
8 The handwash sink in the food prep area was blocked by large trash can and not accessible at all times for employee use.
10 No time/temperature logs available to show that the foods delivered recently were delivered at the proper temperature. Per invoice, food arrived at 10:15 a.m., but were not at the proper temperature at approximately 10:45 a.m.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.
Food employee instructed to take immediate corrective action.
19 Potentially hazardous food was observed at 131 degrees F rather than held at 135 degrees F or above as required. (Food temperatures were not logged by facility personnel at time of delivery.)
Note: The food will be served within the next two hours.
20 -Potentially hazardous food (cut tomatoes) delivered this morning and observed on kitchen counter was 50.7 degrees F rather than 41 degrees F or below as required.
-Cheese delivered this morning and observed on kitchen counter was 54.8 degrees F rather than 41 degrees F or below as required.
-Milk on counter in classroom was 55.7 degrees F rather than 45 degrees F.
Corrective action: Items discarded.
45 -Bare wood shelving observed holding chemical in the mop closet.
Note: Surfaces should be smooth, non-absorbent, durable and easy to clean.
49 -Water in the staff toilet room, in the front on the first floor, took several minutes to dispense water that was 100 degrees F.
-Water in second floor classroom was observed at 95.7 degrees F after handwashing for lunch.

-Facility does not have its own time/temperature records showing the temperature of the food at arrival.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Upon arrival sinks observed set up without sanitizer solution. Upon departure, fresh wash solution and rinse water were observed, but no sanitizer solution was prepared.
20 Potentially hazardous food was held at 52.9 degrees F rather than 41 degrees F or below as required.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Sandwiches delivered in the morning were observed on a cart outside of the food prep area at approximately 12:30 p.m. Snack to be served at approximately 2:30 p.m.
Corrective action: Sandwiches were placed in the refrigerator.
33 Food being thawed using an unapproved method. Food observed thawing at room temperature on a cart outside of the food prep area. Per food employee, sandwiches arrived frozen.

Corrective action: Paper towels provided.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 50 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.

-Milk was held at 48 to 49 degrees F, rather than 45 degrees F or below as required.
Corrective action: Food above 45 was discarded. Dairy above 48 degrees F was discarded. Remaining items were pushed to the rear of the refrigerator and cardboard was removed from shelves in an attempt to improve air circulation.
