1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. No food safety certified person on site. Corrective action: Food safety certified person arrived during report writing.
2 No food safety certified person available to demonstrate knowledge of the foodborne illnesses deemed reportable or the symptoms that would require employee exclusion. Corrective action: Arriving food safety certified person was able to demonstrate knowledge of most illnesses and symptoms.
8 Soap was not observed at the handwashing sink in the Fireflies classroom. Person in charge was instructed to take immediate corrective action.
8 The handwash sink in the food prep area was blocked by large trash can and not accessible at all times for employee use.
10 No time/temperature logs available to show that the foods delivered recently were delivered at the proper temperature. Per invoice, food arrived at 10:15 a.m., but were not at the proper temperature at approximately 10:45 a.m.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Food employee instructed to take immediate corrective action.
19 Potentially hazardous food was observed at 131 degrees F rather than held at 135 degrees F or above as required. (Food temperatures were not logged by facility personnel at time of delivery.) Note: The food will be served within the next two hours.
20 -Potentially hazardous food (cut tomatoes) delivered this morning and observed on kitchen counter was 50.7 degrees F rather than 41 degrees F or below as required. -Cheese delivered this morning and observed on kitchen counter was 54.8 degrees F rather than 41 degrees F or below as required. -Milk on counter in classroom was 55.7 degrees F rather than 45 degrees F. Corrective action: Items discarded.
45 -Bare wood shelving observed holding chemical in the mop closet. Note: Surfaces should be smooth, non-absorbent, durable and easy to clean.
49 -Water in the staff toilet room, in the front on the first floor, took several minutes to dispense water that was 100 degrees F. -Water in second floor classroom was observed at 95.7 degrees F after handwashing for lunch.

45 -Reach-in refrigerator/freezer combination in the infant classroom is holding food above 41 degrees F. Temperature adjusted during the inspection. -(Ref. 46.501) The reach-in refrigerator/freezer combination in the infant classroom is not NSF/ANSI approved.
46 NSF/ANSI approved drying rack is needed for some items on drying shelf to avoid wet nesting and to provide air circulation for drying items. Plates and containers should not be stacked while wet. Note: Towel drying is not approved.
49 -Water at handwashing sinks in two classrooms reached 86 to 90 degrees F rather than 100 degrees F. -Water throughout the building in multiple classrooms took several minutes to reach 100 degrees F.
53 Unapproved ceiling tiles observed in the food prep/warewashing area.
53 -Please continue to organize and declutter the supply/janitor room. -A couple of old mouse droppings were observed in the closet in the Fireflies classroom.

34 Food temperature measuring devices for ensuring proper food temperatures are not calibrated and/or functioning properly. Ice method for testing thermometer accuracy was discussed with the person in charge.

8 Paper towels were not available at the handwashing sink in the food prep area. Adding a paper towel dispenser near the handwashing sink was discussed. Corrective action: Paper towels were found on the counter nearby.
10 -Recently received (sandwiches) foods found unrefrigerated at food facility for an extended period of time. -Facility does not have its own time/temperature records showing the temperature of the food at arrival.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Upon arrival sinks observed set up without sanitizer solution. Upon departure, fresh wash solution and rinse water were observed, but no sanitizer solution was prepared.
20 Potentially hazardous food was held at 52.9 degrees F rather than 41 degrees F or below as required.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food. Sandwiches delivered in the morning were observed on a cart outside of the food prep area at approximately 12:30 p.m. Snack to be served at approximately 2:30 p.m. Corrective action: Sandwiches were placed in the refrigerator.
33 Food being thawed using an unapproved method. Food observed thawing at room temperature on a cart outside of the food prep area. Per food employee, sandwiches arrived frozen.

6 -Hot water at handwashing sink in the Panda Bears classroom only reach 77 degrees F. -Capacity of hot water tank in Panda Bears toilet room is insufficient for number of children washing hands. Water went from 120 degrees F to 99 degrees F within approximately 3 minutes.
8 Paper towel dispenser empty at the handwashing sink in the adult toilet. Corrective action: Paper towels provided.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 50 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions.

14 The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was greater than 200 ppm, rather than 50-100 ppm as required. Concentration corrected during the inspection.
20 -Potentially hazardous food was held at 52 degrees F rather than 41 degrees F or below as required. (Pureed cooked food.) -Milk was held at 48 to 49 degrees F, rather than 45 degrees F or below as required. Corrective action: Food above 45 was discarded. Dairy above 48 degrees F was discarded. Remaining items were pushed to the rear of the refrigerator and cardboard was removed from shelves in an attempt to improve air circulation.
