13 Raw chicken was stored above ready to eat foods in the walk in cooler. *The PIC moved the items to bottom shelf.
13 Foods in the walk in cooler and reach in freezer observed uncovered. *The PIC covered the items during the inspection.
36 An insect control device located in food prep area above the rice cooker with potential to contaminate food, equipment, and / or utensils. *The PIC removed the device.
45 Raw wood shelving observed in the basement storage, and upstairs rear prep area which are not smooth and easily cleanable, being used to support equipment and beverages. Plastic soda crates observed holding up equipment and beverages.
45 -Domestic chest freezers, slow cooker, and reach-in freezers observed. -Front kick plates are missing on both reach in refrigerators in front of food prep area. -Ice build-up observed inside chest/reach-in freezers. -Garbage disposal installed on a shallow sink. -Garbage disposal observed inoperable.

51 -A lidded trash can was not observed in the restroom. -Toilet observed missing the back cover.
53 -Cracked or damaged ceilings observed in rear food prep area. -Base coving observed not sealed to the wall in the employee restroom.

45 -Garbage grinder installed on a shallow wall-mounted handwashing sink. -Domestic chest freezer, and reach-in freezer observed. -The splashguard on the handwashing sink was in poor repair. -Unused prep sink. Sanitarian explained that each sink serves a purpose, prep sink is for cutting vegetables and fruit. three basin sink is for washing, rinsing and sanitizing dishes only. -Ice build-up observed inside chest/reach-in freezers.
46 Food facility lacks the proper sanitizer test strips to monitor for chlorine sanitizer concentration. QAC strips were being used. Sanitarian showed the person in charge the proper strips, how to use them and check for the proper ppms
