1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **Food safety certified person was not present at the time of the sanitrian's arrival. Food safety certified person arrived at the time of the inspection.
19 Pizza was held at 82.6 degrees F rather than 135 degrees F or above as required.
20 Several foods were held at 47.2 to 54.3 degrees F rather than 41 degrees F or below as required. Salmon was discarded, other food items were relocated to walk-in cooler for quick chill
21 Opened commercially processed ready to eat food (deli meat and cheese), located in the walk-in cooler was not marked with the date it was opened. Food employee date marked at the time of the inspection.
22 Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods (pizza) without written procedures or documentation to verify disposition of food. Log was created at the time of the inspection.

1 Certified person was not present at the start of inspection. They later arrived. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 Milkshake blender and ice cream was observed in the front area, but the food facility does not have a hand wash sink conveniently located in the area. Milkshake blender was relocated to the kitchen area and ice cream was removed.
8 Paper towel dispenser empty at the handwash sink in the men's restroom. Paper towels were provided.
14 Slicer has an accumulation of food residue on the food contact surfaces and is not being washed, rinsed and sanitized at least every 4 hours. Slicer was taken apart and cleaned.
14 Observations: - The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was over 400 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. - The warewashing sink was not set up in the correct order (wash, rinse, sanitize).
21 Foods in the walk-in cooler were not date marked with the day it was prepared or opened. Dates were provided.
