41 Ice scoop handle in the smoothie ice bin observed in contact with the ice and not having handle stored above the top of the food.
Provide holster for ice scoop as discussed.
46 Provide new quaternary ammonium sanitizer test strips. Test strips present show signs of having been wet. Provide color comparator to determine concentration.
47 Clean equipment prior to use.
Wash, rinse, sanitize, air dry food contact surfaces, drop in pans, etc. prior to use.
49 Provide caps for potable water fill ports.

6 Food employee did not follow proper handwash procedures. Food employee did not wash hands between changing tasks.
8 The following observations were noted:
-No hot water, water temperature measure at 59 degrees F
-Soap was not available at the handwash sink in the prep area.
**Corrective action was taken, person in charge fixed the hot water issue and provide soap at time of inspection.**
11 Observed fruits and leafy green vegetables stored in mini reach-in refrigerator, smoothie prep juice, protein powder mix and blender with smoothie beverage residue held inside food prep trail unit.
Health Sanitarian advised certified food safety person to discontinue the use and prepping of smoothies beverage and must submit Plan Review Application.
13 Observed raw/uncooked chicken stored in reach-in refrigerator stored above vegetables in the reach-in refrigerator.
13 Observed single-use cup without handle submerged in flour stored in working bulk container.
14 Observed black residue along top internal residue of cooler container use to store ice for customer consumption.
20 Ready-to-eat food (i.e. sliced vegetables) was held at 60 - 125.4 degrees F rather than 41 degrees F or below as required.

50 Observed ice stored in cooler without drain spout with catch tray.
61 Food establishment licensee and operation changes require submission of a Plan Review Application based on observation that food trail unit is being operated by a new owner and smoothie beverages are being sold for public consumption.
