14 Black residue, pink slime observed on the interior of the ice machine bin
Person in charge assigned an employee who emptied all the ice and scrubbed it down to clean it before the close of the inspection
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 150 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions.
Additional pill added to solution to derive the required amount of 200 ppm during the inspection.
20 Cheese and lettuce in cold hold refrigerator food was held at 50 degrees F rather than 41 degrees F or below as required.
Person in charge adjusted temperature on cold hold prep. refrigerator to derived the required temperature of 41 degrees F before the end of the inspection.
39 Moist wiping cloths observed lying on prep tables and not stored in sanitizing solution.
45 -Non-NSF/ANSI certified reach-in freezer and domestic microwave observed during the information.
-Plastic drink crates used for elevation.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.

8 The handwash sink in the warewash/prep area was blocked by boxes on a chair and utensils inside the sink and not accesible at all times for employee use.
All items moved and removed from hand wash sink during the inspection to allow for accessibility.
8 Paper towel dispenser empty at the handwash sink in the prep/warewash area.
Paper towel was replenished during the inspection.

26 -Open rodenticide observed on floor in basement area and between refrigeration unit and counter is POS area.
-Rodenticide was removed.
37 -Beverages and boxed oils are not stored at least 6 inches above the floor in the warewash area.
39 Moist wiping cloths observed lying on prep tables and not stored in sanitizing solution.
41 A serving spoon stored in a container of water by the steam-well observed in stagnant water instead of hot water at 135 degrees Fahrenheit and above or running water.

A food facility without facilities specified in § § 46.711—46.719 (relating to cleaning of equipment and utensils) for cleaning and sanitizing kitchenware and tableware shall provide only single-use kitchenware, single-service articles, single-use articles for use by food employees and single-service articles for use by consumers.
