8 Paper towel dispenser empty at the hand wash sink in the restroom area. Paper towels were provided.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the refrigerator, was not date marked. Dates were provided.
37 Box of apples were not stored at least 6 inches above the floor.
45 Soda crates were observed being used for elevation and cardboard lining shelves in the basement were not smooth and easily cleanable.
45 Observations:
- One unused oven observed in kitchen area.
- An unused fryer in the kitchen
- Domestic cabinetry behind the front counter.
47 Sanitizer bucket was stored in the prep sink. Bucket was removed.
53 The following observations were made:
- Water stained ceiling tiles in retail area and basement
- Gaps observed around piping in the ceiling in the prep area
53 General cleaning:
- Clutter observed in the basement
- Old mouse droppings along the floor perimeter

8 Two tables with chairs were observed for customer seating. Facility does not have a handwash sink accessible for customers. Seating was removed.
36 Mouse droppings were observed in the basement on some shelving and along the floor perimeter.
41 Utensils observed in unclean, stagnant water. Utensils were removed from water.
45 Soda crates were observed being used for elevation and cardboard lining shelves in the basement were not smooth and easily cleanable.
