1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.

1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation.

4 An open employee's beverage container was observed in the hot food preparation area adjacent to the service area.
** Advised to allocate a separate space designated for employee use only or provide coverings for all employee beverages and not to store them in the preparation areas.
6 Food employee did not follow proper handwash procedures. Food employee did not wash hands between changing tasks. -Observed a food employee enter the basement prep area and slice bread without a prior handwash.
Corrective Action: Food employee washed their hands upon request.
13 -Raw shell eggs were observed stored above raw beef and ready to eat foods in the walk-in cooler.
** Advised to organize the foods so that the eggs are on the bottom, with beef above, and all ready to eat foods on top.

14 -Black residue and pink slime observed on the interior of the ice machine bin around the front basin.
** Advised to remove all ice and to wash, rinse, and sanitize the interior of the ice machine.
-Coffee machine at the coffee station observed contaminated by a water leak in the ceiling.
** Advised to discontinue usage of the coffee machine and to fix the leak.
14 Chlorine chemical sanitizer residual detected in the final sanitizer rinse cycle of the low temperature sanitizing dishwasher was 200 ppm, and not 50-100 ppm as required.
** Advised to get the machine maintenanced to ensure it is delivering the correct sanitizer levels.
14 The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by the insertion of a probe thermometer in the machine. Probe thermometer measured a max temperature of 128.5 degrees F.
** Advised to get the machine maintenanced to ensure it is delivering the correct sanitizing temperature levels.
21 Foods prepared in the facility including prepared foods, fruit based purees, repackaged cheeses and an open and partially used gallon of milk not labeled with date markings of the original open or preparation date.

** Advised to remove all of the unapproved glue traps from the facility and to only have pest management services provide pest control in the food facility.
