14 The quaternary ammonium concentration in the sanitizing solution of the sanitizer container was 0 ppm, rather than 200 - 400 ppm as stated on the manufacturer's use directions. Corrective Action: The sanitizing solution was remade and tested at 200 ppm.
45 Non food certified equipment observed: reach-in refrigerator, toaster, air fryer.

Corrective Action: The Philadelphia Food Safety Certificate was distributed. ** Advised that the receipt from dropping off the application at Central Office would be acceptable.
8 Paper towels not present in the restroom. "Each handwashing sink or group of adjacent sinks shall be provided with one or more of the following: (1) Individual, disposable towels.(2) A continuous towel system that supplies the user with a clean towel. and/or (3) A heated-air hand drying device."

14 Observed unapproved chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use (Label not provided-bleach based tablets, the tablets were discarded). ** Advised to obtain Quat tablets or bleach approved for usage on "non-porous food contact surfaces".
26 Observed employees foods stored on the same shelves and above food designated for the children in the reach-in refrigerator. ** Advised to segregate the foods via storing the childrens' food above all food for employees to prevent contamination.
36 Visible physical evidence of rodent (mouse droppings) activity observed in the bathroom, prep, sitting and stair areas. ** Invoice from pest management services observed: Coles Pest Solutions. Advised to request further pest management services and identify exactly where mouse droppings were observed.

20 Cheese was measured at 51.8 degrees F, rather than 45 degrees F or below as required.
26 -Chemicals (lysol wipes) were stored above or on the same shelf with food, equipment, and/or single service articles in the prep area. Corrective Action: the lysol wipes were relocated. -Chemicals and cleaning elements observed stored in the cabinet beneath the sink in the prep area that is accessible to the children without a lock to prevent access. Corrective Action: A lock was placed on the cabinet.
36 Visible physical evidence of rodent (mouse droppings) activity observed in the bathroom, prep, sitting and stair areas. -PIC was unable to provide an invoice or proof of services from a pest control professional.
