4 - Observed a food employee's open container of yogurt (personal food item) in the reach-in refrigeration unit, which is used for consumer products, in the prep area. Corrective action; PIC removed the food employee's personal food item from the reach-in refrigerator during the inspection and agreed to designate a space in the facility for employee's to store their personal food items.
6 Food employees observed donning single use gloves without a prior hand wash. Corrective action; Sanitarian asked food employee to wash their hands before donning single use gloves during the inspection.
8 The paper towel dispenser at the handwashing sink in the kitchen/prep area was empty at the time of inspection. Corrective action; Food employee replenished the paper towels during the inspection.
26 Observed loose rodenticide (blue packet of rodenticide) not labeled by the manufacturer or properly contained/protected on the floor perimeter near the PIC office. Corrective action; PIC removed the loose rodenticide during the inspection.
39 Moist wiping cloths observed lying on equipment and not stored in sanitizing solution.
47 - Observed rust and stains on shelving surfaces in the prep/kitchen areas.

- Observed food debris and grease on the shelving surfaces underneath the cooking equipment in the kitchen/prep area.

- Observed dust and food debris/crumbs underneath the toasting equipment in the prep area.

- Observed dust, debris, and garbage on the floor perimeters and in between shelves in the retail areas.
- Observed, dust, debris, and stains on floor perimeters in the kitchen/prep and basement storage areas.

13 Chicken fingers, fries, and onion rings observed uncovered in the kitchen reach-in freezer.
**Person in Charge (PIC) placed these food items into covered containers during the inspection.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay ware washing sink was 150 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions.
**Person in Charge (PIC) added more tablets to the water, and the concentration of the solution was later measured at 200 ppm.
14 Residue observed on the interior surface of the ice machine bin door.
**Person in Charge (PIC) cleaned and sanitized the ice machine bin door during the inspection.
21 Opened deli meat, located in the deli display case in the kitchen was not marked with the date it was opened.
**PIC provided date sticker during the inspection.
37 -Food (frying oil) is not stored at least 6 inches above the floor near the reach-in freezer in the kitchen.
-Cases of water observed not stored at least 6 inches above the floor at the stairway leading down to the basement.
41 Single-use food container observed without handle and used as a scoop in direct contact with bulk food ingredients in the kitchen.
45 -One of the salad prep units in the kitchen lacks a condensing unit cover.
-Plastic crates observed being used to elevate food storage in the kitchen, retail area, and basement.
45 Cutting boards attached to the salad prep units in the kitchen observed with deep scratches and scoring, not allowing for effective cleaning and sanitizing.
47 -Observed grease build-up on the exterior surfaces of the ice machine in the kitchen.
-Black residue observed on the holding racks inside the "Tropicana" reach-in refrigerator in the kitchen.
-Ice accumulation observed on the interior of the chest freezers in the basement.
51 Dusty ceiling vent cover observed in the restroom.
53 -Grease build-up observed along the baseboard of the walls in the retail area and the kitchen.
-Food debris, black dust, and greasy substances observed on the floor perimeters of the kitchen, retail area, and basement, under multiple food preparation and cooking equipment, under the retail shelving units and basement dry storage units.
53 Maintenance tools (brooms & dustpans) observed stored idle at the basement stairway; relocate to utility storage area when not in use.
