47 The following were observed:

-Food debris/residue on lower interior surfaces of under-the-counter/reach-in refrigeration units

-Grease accumulation on cooking equipment

-3 bay ware-washing is being used as a food prep sink. Food facility lacks food prep sink with an indirect drain to wash produce and raw meats
-Uncovered potentially hazardous foods in walk-in cooler
-Raw chicken stored above sauces in walk-in cooler/beverages in reach-in refrigeration unit
-Raw pork stored above pre-cooked in under-the-counter refrigeration unit
**PIC was issued a variance form during time of inspection.
14 The chlorine concentration in the sanitizing solution of the 3-bay warewash sink was greater then 200 ppm, rather than 50-100 ppm as required.
*PIC took corrective action and diluted sanitizer concentration to 50-100 ppm as required.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler, reach-in refrigeration unit, and under-the-counter refrigeration unit was not date marked.

**No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
**Food Safety Certified person arrived by end of the inspection.
11 Three dented cans observed on shelving unit in food prep area.
**PIC was instructed to discard dented cans during time of inspection.
13 The following were observed:

26 Working containers in food prep area, used for storing chemicals taken from bulk supplies, were not marked with the common name of the chemical.
**PIC was instructed to label chemical bottles during time of inspection.
35 Working bulk food ingredient storage containers are not labeled with the common name of the food.

-Container of raw chicken stored on floor under ware-washing station next to chemical items
-Several uncovered potentially hazardous food items in reach-in refrigeration units, under-the-counter refrigeration units and walk-in cooler
**PIC was instructed to discard raw chicken and to properly cover all potentially hazardous food items during time of inspection. (COS)
26 Chemicals were stored directly next to food (raw chicken) on floor under ware-washing station in the food prep area.
**PIC was instructed to discard raw chicken and properly store food in designated area for food during time of inspection.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible in some under-the-counter and reach-in refrigeration units.
36 Visible physical evidence of insect activity (mouse feces) observed in utility area near unisex employee restroom area.
