-Damaged gasket observed at the walk-in cooler door.
49 - Leacky faucet observed at the prep sink/kitchen area.
51 - "Out of service" toilet (one) observed in the woman's restroom.

-Hot holding unit does not maintain proper hot holding temperature. Ambient air temperature observed at 108-110 degrees F.

-Light food debris on exterior surfaces of shelving units
-Light food debris on lower interior surfaces of hot holding units
-Light food debris on lower interior surfaces under-the-counter refrigeration units
-Light food debris around perimeters of floors in walk-in cooler/freezer
48 One to two minute delay in hot water reaching peak demand at handwashing sink in ware-washing area.

**PIC took corrective action, discarded old mouse feces and sanitized floors during time of inspection.

11 The following were observed:
-Potentially hazardous food was held at 55-65 degrees F rather than 41 degrees F or below as required in cold prep/under-the-counter refrigeration unit in front food service line.
-Potentially hazardous food was held at 110-115 degrees F rather than 135 degrees F or above as required in hot holding unit in food prep area.
**PIC was instructed to discard all potentially hazardous food during time of inspection and store foods in units that maintain proper cold and hot holding temperatures until serviced by maintenance.
14 Black residue, pink slime observed on the interior of the ice machine.
**PIC was instructed to discard ice and properly wash, rinse, and sanitize interior surfaces of ice machine during time of inspection.
19 Potentially hazardous food was held at 110-115 degrees F rather than 135 degrees F or above as required in hot holding unit in food prep area.
**PIC was instructed to discard potentially hazardous food and store food in a hot holding unit that maintains proper hot holding temperature.
20 Potentially hazardous food was held at 55-65 degrees F rather than 41 degrees F or below as required in cold prep/under-the-counter refrigeration unit in front food service line.
**PIC was instructed to discard potentially hazardous food and store in a refrigeration unit that maintains proper cold holding temperature.
21 Potentially hazardous ready to eat food, prepared in the food facility and held for more than 48 hours, located in the walk-in cooler and under-the-counter refrigeration unit, were not date marked.
**PIC took corrective action and date marked all potentially hazardous food during time of inspection.

-Cover for indirect drain observed missing under exhaust hood and mechanical ware-washing machine

-Ice accumulation on perimeters of ceiling in walk-in freezer
-Food debris on exterior surfaces of shelving units
-Food debris on lower interior surfaces of hot holding units
-Food debris on lower interior surfaces under-the-counter refrigeration units
-Light food debris around perimeters of floors in walk-in cooler/freezer

51 One stall observed out of order in women's employee restroom area.

1 No City of Philadelphia Food Safety Certificate provided, ServSafe course certificate only. Person submitted the application for the Food Safety Certification, no receipt.
11 A dented can of "Red Pack" pizza sauce, 106 oz. was observed in the storage room.
Person in charge discarded the dented can during inspection.
14 The can opener had an accumulation of food residue on the food contact surfaces.
**Sanitarian advised the person in charge to washed, rinsed, and sanitized the can opener.
14 The food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher as evidenced by the failure of the temperature indicator. The food-contact surfaces of equipment reach 152 degreed F.
Person in charge took unit out of service and will utilize the 3-bay sink for washing, rinsing, and sanitizing the food equipment until the dishwasher will be adjusted.
36 Visible physical evidence of rodent (mouse droppings) observed along the floor perimeters in the storage area.
45 Plastic crates being used for food elevation in the walk-in freezer.
45 -Unused equipment observed in food prep and serving areas.
-Some cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 -Ice build-up inside the chest freezer in the cooking area.
-Dust accumulation on the outside filter of the ice machine.
48 - A hot water delay was observed at the handwashing sink located in the employee restroom area. It took approximately 5 minutes before the hot water was at proper temperature.
49 The service date on the water filter from ice machine was 2020.
53 The following observations were noted:
- Debris buildup observed under the dishwasher machine in the warewashing area.
- Mouse droppings observed on floor surface in the storage room.
53 Brooms and dustpans observed stored on direct contact of floor surface in the food prep area and storage area.
53 -Unsealed utility line penetrations were observed in the warewashing area and storage area.
-Damages ceiling tiles observed in the storage area.
