1 The food facility operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. Food Safety Certified Person arrived in middle of inspection.
4 An open employee's beverage container was observed in a food preparation area.
13 Raw chicken observed stored over container of milk/cream.
19 Sauce in steam table was observed at 98 degrees F. Remove at reheat to proper temperature prior to storage in hot holding unit.
22 Time in lieu of temperature being used as a control for potentially hazardous foods, but no documentation was provided at time of inspection for Pizza slices stored at front serving counter.
36 Mouse feces observed in corner behind unused reachin refrigerator near soda syrup rack unit in rear storage area
45 -unused equipment observed in rear of facility including dough mixture -inoperable reachin unit observed used for dry storage in rear of facility
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
47 Heavy ice accumulation observed in chest freezer units.
53 Floor cleaning and disinfecting is needed where mouse feces are present in above-mentioned area
