1 Certified person did not have a city certificate. Application given to the person in charge.
No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 The food facility does not have a handwash sink conveniently located in the prep area (near the cook line/steam table).
13 Several raw animal foods were stored above ready to eat foods in the walk-in cooler and refrigerator. Person in charge began rearranging foods.
13 Beverage service lines were observed in contact with drinking ice.
14 Square cornered 3-basin warewashing sink observed with accumulation of residue in the corners.
26 Chemicals were stored above or on the same shelf with food, equipment, and/or single service articles.
36 Flies observed in the dishwasher area/prep area.
37 Beverage service lines were observed in contact with drinking ice.
Note: Ice used as exterior coolant is prohibited as ingredient. Ice may not be used as food after it has been used as a medium for cooling the exterior surfaces of food such as melons or fish, packaged foods such as canned beverages or cooling coils and tubes of equipment.
41 Common bowls or food containers observed without handles in direct contact with bulk food (sugar, flour) ingredients.
Food dispensing utensil in the ice observed in direct contact with food and not having handle stored above the top of the food.
45 Garbage disposal was observed on the wash compartment of the 3 basin sink.
45 The following observations were noted:
-Non-NSF/ANSI approved handwashing sink, freezer, square cornered warewashing sink
-Defective walk-in cooler being used for dry storage
-Floor uneven and in disrepair in the storage room and walk-in cooler.
45 Raw wood pallets being used for elevation in the basement were not smooth and easily cleanable.
50 Air gap was removed from the indirect drain under the food prep sink. Person in charge removed the rubber from around the pipe.
53 Cleaning tools (mops) observed in direct contact of the floor and not elevated or hung on a utility rack.
53 General cleaning needed:
- Stained carpeting in the defective walk-in cooler
- Food residue/debris observed around equipment in the prep area
