1 No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
Note: PIC will renew Department issued certificate.
13 Several uncovered food observed in units throughout facility. All food must be covered prior to storing.
21 Cooked and cut potentially hazardous ready to eat food(lobster, lettuce, ..) prepared in the food facility and held for more than 48 hours, located in the Cold holding unit, were not date marked with the dates they were cooked/cut.

46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
54 Vent above the gill and inside walk-in cooler was observed with grease and dust. Cleaning needed.

6 Food employees observed donning single use gloves without a prior hand wash.
Note: Employees must wash hands prior to wearing a glove.
7 A food employee was observed touching ready to eat food with his bare hands.
Note: Food employee was observed cutting tomatoes with his bare hands.
8 Cleaning solution bottles were stored on the hand wash sink located in food prep area.
Note: Bottles were removed from hand wash sink.
8 The handwash sink in the food prep area was blocked by crates and not accessible at all times for employee use.
Note: Crates were removed during time of inspection.
13 Several uncovered food were observed in the cold prep unit located in the pizza prep area.
Note: All food must be covered or properly wrapped prior to storing.
21 Potentially hazardous opened commercially processed ready to eat food(deli meat, cheeses,) located in the walk-in and cold prep unit, and held more than 48 hours, were not marked with the date they were opened.
Note: Date marking provided during time of inspection.
36 Outer opening in the food facility does not protect against the entry of insects, rodents, and other animals.
Note: Side door to the food prep area was observed opened.
37 Food is not stored at least 6 inches above the floor.
Note: Bottles of water observed stored directly on the floor in the walk-in cooler.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
39 Moist wiping cloths observed lying on table and not stored in sanitizing solution.
