14 An accumulation of grease observed on the food contact surfaces of the drying rack in the warewash area.
21 Opened commercially processed ready to eat foods (deli meat and cheese), located in the closed display case and walk-in cooler, was not marked with the date it was opened. PIC date marked the item at the time of the inspection.
26 Chemical spray bottle was stored above or on the same shelf with food, equipment, and/or single service articles in the prep area. PIC relocated the spray bottle at the time of the inspection.
38 Food employees observed not wearing or wearing ineffective hair restraints, such as nets, hats, or beard restraints.
41 Knives observed stored between cold prep unit and prep table which is not easily cleaned and sanitized.
45 - NON-ANSI/NSF sanitation approved chest freezers in retail area
- Unused equipment/griddle observed on lower shelving units in food prep area
- Observed crack in the glass sliding door of the closed display case

- Black residue observed on gasket in the chest freezer.
48 No hot water observed at the handwash sink in the prep area. PIC turned the hot water on from the shut off valve. Hot water is now restored.
49 Condensation lines observed leaking into bucket in the basement area
53 - Grease accumulation was observed on the wall behind the convection oven and on the ceiling in the prep area
- Food residue observed on the ceiling in the prep area
- Debris observed on the floor in the walk-in cooler
- Dead cockroaches observed on the floor in basement area
61 The facility was approved for cold sandwiches. Egg and bacon was observed on the menu . A unused griddle was observed on the bottom of the prep table in the prep area. Griddle and any unapproved equipment must be removed from the establishment.

13 - A pan of chicken cutlets was observed uncovered on the retail counter. The pan of chicken cutlets was relocated during the inspection.
- A box of bread was observed uncovered on the retail counter. Food employee covered the bread during the inspection.
14 An accumulation of grease observed on the food contact surfaces of the drying rack in the warewash area.
20 Chicken was held at 49.1 degrees F rather than 41 degrees F or below as required. PIC relocated the food item to the walk-in cooler for a quick chill.
34 Temperature measuring devices for ensuring proper temperatures of food are not available or readily accessible.

54 There is no exhaust hood above the convection oven in the prep area. Grease accumulation was observed on the surface of convection oven, on the adjacent wall and ceiling surfaces. (Homemade chicken cutlets was observed on the menu and a pan of chicken cutlets was observed in the cold holding unit)
Note: the convection oven was approved to bake pretzels and breads that will not generate grease and smoke due to lack of exhaust hood.

No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
13 Self-serve, unwrapped pretzels were observed uncovered on the counter. Container was covered.
14 Live roach observed inside a pack of lids in the basement. Pack of lids were discarded.
36 Visible physical evidence of insect activity observed:

- Dead roach carcasses observed throughout shelving and floor surfaces in the basement. Cleaning is needed.
36 Gaps were observed under front door in retail area in the food facility which does which not protect against the entry of insects, rodents, and other animals.
