13 Raw animal food stored above ready to eat foods. Bowl with raw egg residual was observed stored in the upper compartment of the cold prep refrigerator above uncovered sandwich peppers. Corrective action: Bowl relocated and peppers discarded.
45 Crates observed used for elevation and storage of produce.
45 -Wall-mounted, shallow-basin handwashing style sink used as a food prep sink. Food prep sinks should be NSF approved, deep-basin sinks with 4 legs. (Appropriate air-gap was observed already in place.) -Piece of a crate was observed covering the interior components of the cold prep refrigerator.
47 Cleaning required: -Residue observed in the bottom interior of the reach-in freezer in food prep area. -Some dust observed in the fryer cabinet. -Some food debris observed beneath the flat grill. -Gaskets of the cold prep refrigerator require cleaning. -Crate used to cover the front interior mechanical components of the cold prep refrigerator is soiled. -Accumulation of ice observed in the ice cream freezer in the front retail area. -Old mouse feces was observed in hollow space between the bottom shelves in the rear storage area.

20 Potentially hazardous food was held at 43.4 to 46.4 degrees F rather than 41 degrees F or below as required. Corrective action: Cold display case motor was turned on during the inspection. ****Note: Please ensure that refrigeration units are not going into defrost too often or for too long.***** Upon returning to the store for exit interview, cold display refrigerator was in defrost mode again. Instructed the owner to move all potentially hazardous items to colder units prior to leaving the establishment.
26 Rodent bait observed behind the wall between the basement door and the refrigeration unit and in wall across from the basement stairs. Note: Rodent bait should be contained in a covered, tamper-resistant bait station.

11 One gallon of expired milk was observed in the reach-in refrigerator in the retail area. Corrective action: Milk discarded.
13 Knife for cutting sandwiches was observed stored on cardboard in the cold prep refrigerator. Corrective action: Knife was moved to the warewashing sinks and cardboard was discarded.
19 Potentially hazardous food was held at 120 degrees F rather than 135 degrees F or above as required. Corrective action: Temperature of the unit where the recently prepared food was stored was increased.
26 -Observed insecticides or rodenticides not labeled by the manufacturer for food facility usage, in the rear storage area on shelf near chest freezer. Two cans of Raid observed in the rear storage room with other chemicals in use. One can of Raid observed in basement. Corrective action: Items were discarded. Per owner, items are not in use. Schedule for exterminator observed posted. -Rodent bait behind the chest freezer in the retail area was observed in a plastic baggy rather than in a covered, tamper-resistant bait station.

14 Slicer had some food residue on the food contact surfaces and has not been cleaned within the last 4 hours. Slicer must be washed, rinsed and sanitized at least every 4 hours. -Slicer was taken apart and parts moved to the warewashing sink for cleaning.
14 The quaternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 150 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. -Corrective action: Concentration was corrected during the inspection, and proper test strip use, correct sanitizer preparation and proper signage arrangement were all discussed during the inspection.
20 Potentially hazardous food was held at 47.7 to 52.2 degrees F rather than 41 degrees F or below as required. Corrective action: Items were discarded.
26 Observed insecticides or rodenticides not labeled by the manufacturer for food facility usage, in the establishment. -Raid observed in the point-of-sale (POS) area. -Loose packets of rodent bait were observed on the floor, compartment in basement wall in front of the basement stairs, and in the water meter access area. Corrective action: Raid and loose rodent bait removed. Owner will get a mechanical picker/grabber to retrieve the poison near the water meter.
