6 Handwashing sink in the basement warewashing area could not be used due to an inoperable mechanical stopper. Corrective action: Stopper was removed during the inspection.
8 The handwash sink in the food prep area was blocked by a crate and not accessible at all times for employee use. Corrective action: Sanitarian removed the crate and instructed the person in charge not to block the handwashing sink.
9 Food facility Person in Charge not able to provide documentation that foods (soft pretzels) are from an approved source. Corrective action: Pretzels discarded.
13 Uncovered sugar observed at coffee station in the food prep area. Corrective action: Sugar covered.
14 -Wiping cloth, pail of soap and water and scouring pad observed in food prep sink. Corrective action: Person in charge instructed to wash, rinse and sanitize the sink. -Black residue, pink slime observed on the interior of the ice machine bin components. Corrective action: Cleaning initiated during the inspection.
20 Potentially hazardous food (half and half, cheese, lettuce, mayo) was held at 44.9 to 74.6 degrees F rather than 41 degrees F or below as required. Corrective action: Half & half, cheese, mayo and cut tomato were discarded. Instructed the person in charge to decrease the temperature of the closed display case and to move the food until the case reaches the proper temperature.
21 Opened commercially processed ready to eat food (deli meat & cheeses), located in the reach-in refrigerator/freezer combination, was not marked with the date it was opened. Person in charge instructed to date-mark the items.
26 Quaternary ammonia sanitizer solution was prepared in excess of 500 ppm. Corrective action: concentration corrected
36 Rear outer opening in the food facility does not protect against the entry of insects, rodents, and other animals. Large gap at the bottom of door.

45 -(Ref. 46.501) Reach-in refrigerator/freezer unit, chest freezers, porcelain handwashing sink in the food prep area, handwashing sink installed in the warewashing area and ice machine are not NSF/ANSI approved equipment. -Cold prep refrigerator is not holding food at 41 degrees F or below. -Deli case is not holding food at 41 degrees F or below. -Multiple reach-in beverage refrigerators are holding beverages above 41 degree F. -Condensation line for the ice machine was observed emptying into the handwashing sink in the warewashing area. -A cover was not observed on the sump pump well. -Inoperable walk-in refrigerator used for dry storage. -Crates used to elevate food and drink. -Rust observed in warewashings sink basins and on warewashing sink drain covers.
45 Basement storage shelving are bare wood and are not non-absorbent and easy to clean.
47 -Some ice accumulation and debris were observed in the chest freezer in the retail area. -Gaskets and exterior of basement chest freezer observed soiled. -Black residue observed on the fan cover in the reach-in milk refrigerator. -Clutter observed on shelving in the point of sale area. -Accumulation of dust observed on the ceiling fan in the retail area.
49 -Water observed exiting pipe in basement toilet room when pump came on while testing water temperature. -Sink stopper at the handwashing sink in the basement was stuck and the sink could not be used. Stopper was removed during the inspection.
52 Full trash bags were observed on the floor in the rear shed. Trash must be contained in trash cans with tight fitting lids.
53 -Extreme odor in basement. -Unnecessary items observed in the basement. -Black, mold-like substance was observed on drywall in the rear of the basement.
54 The lighting intensity above the warewashing sinks is inadequate. Less than 50 lux rather than 220 lux.
54 Lightbulb added to warewashing area is not shielded from breakage.

45 -Reach-in refrigerator/freezer unit, chest freezers, porcelain handwashing sink in the food prep area, handwashing sink installed in the warewashing area and ice machine are not NSF/ANSI approved equipment. -Gaskets on some of the refrigeration units are in disrepair. -Reach-in refrigerator observed in prep area is inoperable. -Top of the cold prep refrigerator was holding food above 41 degrees F during the inspection. -Condensation line for the ice machine was observed emptying into the wash compartment of the warewashing sink. Condensation line was elevated above the handwashing sink basin until a drain can be provided. -A cover was not observed on the sump pump well.

8 Please provide a trash receptacle for the disposal of paper towels near the handwashing sink in the warewashing area.
14 -Black residue, pink slime observed on the interior of the ice machine bin on plastic components. Corrective action: Person in charge was asked to discard ice and to wash, rinse, sanitize and air dry the ice machine. Cleaning was initiated during the inspection. -Slicer guard soiled and a crack was observed in the plastic that holds the meat in place. Residue observed in the cracked plastic.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 to 100 ppm, rather than 200 to 400 ppm as stated on the manufacturer's use directions. Fresh sanitizer was prepared and the concentration was incorrect.
26 New quaternary sanitizer solution prepared while sanitarian was typing report was being used in excess of 500 ppm on food contact surfaces. Sanitarian discussed test strip use and went over the manufacturer's use instructions stating proper concentration of solution and explained how to compare the test strip to the color chart. Corrective action: Quaternary ammonia concentration was corrected.

8 -The handwash sink in the warewashing area does not have paper towels. -No sign or poster posted at the handwash sink in the food prep and warewashing areas to remind food employees to wash their hands. -Soap was not available at the handwash sink in the warewashing area. Corrective action: All items were provided during the inspection.
13 An open container of sugar was observed at the coffee station. Corrective action: Sugar was covered.

8 Slicer parts and a container were observed in the hand-washing sink in the food prep area and the sink was not accessible at all times for employee use. Person in charge was instructed to take the items to the warewashing sink for proper washing and sanitizing. Corrective action: Items were removed.
8 The food facility does not have a handwash sink conveniently located in the ware-washing area. Note: A handwashing sink needs to be installed within the next 30 days.
13 -Washed slicer part was observed being placed on a box in the basement. -Slicer parts observed being washed and sanitized in heavily soiled warewashing sinks. Person in charge was instructed not to use the slicer until the sinks ae properly cleaned and sanitized and new solutions are prepared.
14 Observed no chemical sanitizer being used on equipment and utensils after washing and rinsing them, and prior to use. Slicer observed being washed in the handwashing sink. Slicer moved to warewashing sinks where sanitizer solution was not in use. Instructed the person in charge to prepare sanitizer solution.
14 The quarternary ammonia concentration in the sanitizing solution of the 3-bay warewash sink was 0 ppm, rather than 200 ppm as stated on the manufacturer's use directions. Note: Sanitizer tablets never melted. Quaternary ammonia tablets must be prepared with water that is at least 75 degrees F.
20 Potentially hazardous food (lettuce, tomatoes, mayo and cheese) was held at 50 to 57.2 degrees F rather than 41 degrees F or below as required. Corrective action: Items were discarded. PIC was instructed not to use the unit.
26 -Bleach observed on the same shelf with cereal. -Automotive items observed above pet food. Corrective action: Items relocated.
36 Visible physical evidence of rodent activity observed in the rear storage soda area. Mouse feces was observed on the floor beneath the soda storage racks.
