39 Moist wiping cloths were observed lying on sink counters and not stored in a sanitizing solution.
45 Crates were used to invert items.
46 The three-compartment sink was unclean to sight and touch.
47 Unclean surfaces were observed in the rear area.
53 Brooms and dustpans were observed on the floor.
53 Clutter was observed in the ware wash and rear storage area.

1 Certified person did not have a Philadelphia certificate. Application given to the person in charge. No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department.
8 The handwash sink in the rear area was blocked by boxes/containers and not accessible at all times for employee use. Items were removed.
37 Food boxes/beverage cases were not elevated 6 inches of the floor.
45 Raw wood shelving in the rear area were not smooth and easily cleanable.
47 Observations: - Debris observed under shelving in the walk-in box - Food residue/spillage observed on soda stream in the rear and walk-in cooler - Residue on the exterior surfaces of the soda machine nozzle
51 - Dust accumulation observed on vent cover in the bathroom area. - Dust/debris observed along floor perimeters of the restroom.
53 Peeled ceiling observed in the rear area.
53 Unnecessary clutter observed in the ware wash and rear storage area. Puddles of soda observed on the floor in the rear.

8 Paper towel dispenser stuck at the handwash sink in the front prep area. Paper towels were provided.
11 Expired milk (7/14/22) was observed in the retail area. Milk was removed from sale.
14 Black residue/slime observed on the interior surface of the ice nozzle. Nozzle was taken apart, cleaned and sanitized.
45 Crates being used for elevation in the walk-in cooler and raw wood shelving in the rear area were not smooth and easily cleanable.
47 Observations: - Ice accumulation in the reach-in freezer in the walk-in box - Debris observed under shelving in the walk-in box - Food residue/spillage observed on soda stream in the rear and walk-in cooler
51 - Dust accumulation observed on vent cover in the bathroom area. - Dust/debris observed along floor perimeters of the restroom.
