8 The handwash sink in the food prep area was blocked by a trash can and not accesible at all times for employee use.
36 Visible physical evidence of rodent (mouse Droppings) activity observed in the whole kitchen and warewash area.
45 Cutting boards observed with deep scratches and scoring not allowing for effective cleaning and sanitizing.
45 - Several non-NSF/ANSI approved equipment and not-in-use equipment (Coffee machine, ice machine) was observed in the food prep area.
46 Food facility lacks sanitizer test strips to monitor sanitizer concentration.
47 The following were observed: -Food debris on lower interior surfaces of refrigeration units - Heavy Grease and food accumulation on interior surfaces of cooking equipment and exterior surfaces -Food debris on lower exterior surfaces of shelving units -Rusted shelving units

53 Ceiling tiles observed missing and utility lines have gaps.

1 Food Safety Certified person observed without City Certificate. **No person shall operate a food establishment that handles, prepares or serves non-prepackaged potentially hazardous food who does not have present and in the employ of the food establishment at all hours of operation at least one (1) person with a valid Food Establishment Personnel Food Safety Certificate issued by the Department. **PIC was issued an application for City Certificate during time of inspection.
8 Paper towel dispenser empty at the handwashing sink in the area. **PIC took corrective action and provided paper towel during time of inspection.

49 Leak observed at one toilet in both men's and women's employee restroom area.
53 Food debris observed around perimeters of floors throughout food prep area.
