43 Take-out containers were not properly inverted in the food preparation area.
45 Several washing stations were not properly sealed to the wall.
45 The following observations were noted:
-An unused ice machine in the basement storage area.
-Duct tape was being used around one chest freezer located in the basement food preparation area.
47 The following observations were noted:
-Rust around shelves in several refrigeration units.
-The perimeters of the floor of the walk-in cooler was in need of cleaning due to food debris.
53 The following observations were noted:
-Porous ceiling tiles in the basement food preparation area.
-Openings in the ceiling in the basement area.
53 Water stained ceiling tiles were observed in the basement preparation area.
53 General floor cleaning was needed under and around equipment in the front service food preparation area due to grease build-up and food debris.

41 Knives were being stored between table edges which are not easily cleaned and sanitized in the food preparation area. The person in charge removed the knives to be properly washed.

-The garbage grinder located in the basement area was in need of general repair.

-Rust around shelves in several refrigeration units.
48 A hot water delay was observed at the handwashing sink located in the basement food preparation area. The hot water took several minutes to reach peak demand.
53 Cob webs were observed in the basement storage area and in need of cleaning.

-Openings around the wall near the basement stair case.
-Openings in the ceiling in the basement area.

26 Open bait was observed on the floor in the basement storage area. Rodent bait shall be contained in a covered, tamper-resistant bait station. The person in charge swept/discarded the open bait.

-The interior of one chest freezer located in the basement preparation area was in need of general repair.
-The faucet of the food preparation sink located in the basement area was in need of general repair.

21 Opened commercially processed ready to eat food (cheeses), located in the basement walk-in cooler, and held more than 48 hours, was not marked with the date it was opened. The person in charge took corrective action and date marked the cheeses.
37 Food containers were not being stored at least 6 inches above the floor in the walk-in cooler.

45 The following observations were noted:
