1 The facility's food safety certified handler lacks the City of Philadelphia food safety certificate. Observe a ServSafe certificate only. Advised facility's food safety certified handler to apply for the City of Philadelphia food safety certificate. *Application form issue today 2/12/2025.
8 -Missing hand wash sings in all the restrooms (basement and main bar area). -Paper towel dispenser empty at the hand wash sink in the basement area.
13 Uncovered hamburgers in contact with plastic container observed in the reach-in freezer basement area. PIC removed the hamburgers from the reach-in freezer.
13 Chicken wings, sliced vegetables, meatballs, red sauce, deserts observed stored uncovered in food prep area reach-in refrigeration, walk-in cooler, walk-in, and under counter refrigerator.
14 The quaternary ammonium concentration in the sanitizing solution of the 3-bay warewash sink was 500 ppm, rather than 200-400 ppm as stated on the manufacturer's use directions. *PIC initiated corrective action and lowered the concentration below 400 ppm.

8 Missing paper towels at the hand wash sink kitchen area, PIC provided paper towels during inspection.
9 Food facility Person in Charge not able to provide documentation that slices of cake are from an approved source.
13 Uncovered ice observed in the ice bin bar area.
19 Rice was held at 76 degrees F rather than 135 degrees F or above as required. *Sanitarian instructed the person in charge to discarded the rice.
20 -Food (fried fish, cooked plantain, sauces, dice tomatoes, shrimp tempura, sliced onion) were held at between 57 to 60 degrees F rather than 41 degrees F or below as required in two cold prep units and one under-counter refrigerator kitchen area. *Sanitarian instructed the person in charge to discarded the the food that was hold between 58 to 60 degrees F.

58 Pizza oven was removed from facility. New cooking equipment in the kitchen. Alteration or construction began prior to plan submission and approval. A completed Plan Review Application with proper plans, drawn to scale, showing all design features, including all food service equipment specifications and the location of toilet rooms, plumbing fixtures, storage areas, and utilities must be submitted to the, Office of Food Protection.
